Job overview

We are currently looking at recruiting a Band 7 Urgent Care Patient Flow Clinician. The main role of the post holder is to provide management and leadership responsibility for the effective operational and clinical running of the bed management team (Central Flow Hub). Post holder will have responsibility regarding the trust bed management and ensure it is managed in a safe manner that doesn’t compromise client’s safety.

The team is overseen and further supported by the Urgent Care Lead and SMT. You will work closely with local borough Community Access Service  and with Single Point of Access

The Central Patient Flow Team supports local boroughs by providing a centralised approach to managing inpatient beds and ensuring that all potential admissions are gatekept. The Central Patient Flow Team works closely linked with local Boroughs and responsible for screening and triaging all gatekeeping and bed requests for CNWL. If they accept a referral and face to face Gatekeeping is required, they will liaise with the appropriate borough first response team to conduct the assessment.

Main duties of the job

The post holder will work within Urgent Care Service, provide management and leadership responsibility for the effective operational and clinical running of the bed management team (Central Flow Hub).

Post holder would have responsibility for bed utilisation, ensuring that the demands of bed requests and admissions are balanced across Trust inpatient sites / wards. They would ensure escalation of capacity issues and then carrying out actions to resolve issues as part of the Trust Escalation protocol. 

Post holder would ensure issues regarding bed management are communicated in a timely, effective and constructive way ensuring effective clinical delivery and performance management against key performance indicators and financial targets. 

The post holder will work with the Urgent Care Team Senior Management Team to deliver the vision of the Trust to provide a quality led service. Post holder will work alongside the Urgent Care Senior Patient Flow Lead and Deputy of Urgent Care Service to ensure effective patient flow and bed management for the usage of the Beds across the whole Trust.

Detailed job description and main responsibilities

Bed Management:

  • To provide accurate and timely information relating to the Trust Bed Capacity.
  • Co-ordinating all admissions and transfers across the trust
  • Predicting the need for escalation and organise meetings with all relevant personnel.
  • Predicting any short fall in elective bed capacity and discuss at the escalation meeting. Action any plan as a result and inform all relevant people of the outcome.
  • To ensure repatriation of patients transferred to other hospitals in line with the bed policy.
  • To provide operational management functions for Central Flow Hub
  • Day to day responsibility for bed utilisation, ensuring that the demands of bed requests and admissions are balanced across Trust inpatient sites / wards.
  • Escalation of capacity issues and then carrying out actions to resolve issues as part of the Trust Escalation Plan
  • Monitor usage of out of area and specialist placements, whilst maintaining accurate documentation
  • Ensure issues regarding bed management are communicated in a timely, effective and constructive way
  • Ensuring that capacity management processes are adhered to in order to ensure a high quality and seamless pathway for the patient journey
  • Building and maintaining high quality working relationships with all services that have contact with the bed management service.
  • Proactive response to predicted and actual changes in capacity demand to ensure optimum use of available capacity.
  • Monitoring and liaison with all OOA providers that patients are currently under their service and accurate documentation of progress.
  • Complete all relevant documentation accurately and in a timely manner regarding referrals for beds within the Trust.
  • Provide regular updates on current bed management demand and capacity.
  • Coordination of admissions and discharges across the Trust.
  • Coordination of all transfers of patients to and from Trust wards both internal and external.
  • Provide a visible, accessible and authoritative presence to all staff associated with Central Flow Hub flow
  • To support and develop the strategic direction for the Urgent Care Service in line with national policies and locally identified health needs
  •   To manage a range of processes and systems, across the service to support the effective running of the service and clinical care pathways.
  • To ensure harmonisation of quality care delivery across the service areas.
  • To lead the learning and development plan for services and ensure that developmental requirements are in place for all staff  in  line with the knowledge and skills framework

Management of Service Delivery and Service Development:

  • Establish and develop a professional working relationship with Acute Hospital HCP’s and Management, colleagues and other health and social care professionals.
  • To ensure 24 hour provision of service, through the use of proactive site management, patient flow and capacity utilisation, in line with Trust and national guidelines.
  • To ensure the delivery of clinical and care quality standards and objectives
  • To be responsible for implementing service changes when required, ensuring effective implementation with measurable outcomes and  agreed timescales
  • To ensure that systems are in place to monitor stakeholders views of services and ensure that service users, carers, relevant community organisations and other key stakeholders are involved in and influence key decisions on service development and continuous improvement
  • To participate in the evaluation of the services provided within Mental Health Services and actively promote service development including undertaking relevant audit and research.
  • To ensure that the service area contributes fully to the development of the business plan for the Division and assists in the setting of strategic objectives for the Service
  • To ensure the service area is kept up to date with legislative and policy changes and developments
  • Contribute to the establishment of new services that meet national and local requirements
  • To facilitate the development of clinical guidelines, protocols and patient-related policies in partnership with clinicians and other managers within the organisation to promote safe and effective patient care.
  • Provide information to support the Division Managing Director and Head of Urgent Care and other senior managers in the development of strategy for patient flow  within the organization.
  • To ensure the Patient Flow Team can identify patients whose discharge can be expedited and proactively expedite / escalate issues in ‘real time’

Performance Management

  • To monitor and maintain the standard of information included in the Site Reports and maintain the Site Report distribution list. To regularly review the site information required by the stakeholders.
  • Lead on all breaches including 12 hours A&E by monitoring and producing reports and instigate action plans to mitigate future breach.
  • To support the delivery of single sex accommodation for patients and to accurately report.
  • Liaise with members of the multi-professional team both within the organisation and with external stakeholders, to ensure continuity is maintained and patient flow is maximised; liaising with system managers / the Trust on call team as required.
  • Provide regular recording, monitoring and reporting of Patient Flow Team performance including BUI, bed state, DTAs, ECR, etc. against local and national targets with assurance that appropriate follow up actions will be completed.
  • To adhere to the Trusts Capacity & Demand Escalation Plan. Instigate the use of relevant Action Cards, following discussion with the relevant on-call.
  • Create and embed standard documentation for site handover to ensure consistency and transparency and to document decision making.

Communication

  • To ensure effective communication across the services and with external partners and other stakeholders
  • To be responsible for ensuring a full and timely response to all complaints and ensure that lessons from these complaints are disseminated within the service.
  • Undertake investigations, write investigation reports and feed back to service, service users and carers as appropriate
  • To develop and sustain relationship management with key stakeholders and partner agencies, such as commissioners, local authorities,  Acute Trust, GPs, service user and carer groups, voluntary sector organisations
  • To promote the Trust’s services and positively enhance the Trust’s corporate image internally and with external agencies
  • To ensure patient/client records are in line with standards of practice and Trust policy.

Education/Qualification

Essential criteria
  • Registered Mental Health Nurse, social worker or Allied Health professional with and registration to practice
  • Teaching and Assessing qualification

Experience and Knowledge

Essential criteria
  • Significant Acute Mental health clinical experience
  • Significant experience in working in patient flow/bed management
  • Very good working knowledge of working with London Compact and Who Pay Guidance
  • Good understanding of drivers and constraints around effective bed management.
  • Ability to demonstrate both managerial and clinical skills in previous posts/s.
  • Experience of working with a wide variety of client groups.
  • Demonstrable and considerable experience of undertaking a clinical lead role.
  • Demonstrable experience of effective leadership, role modelling and effective line management.
  • Experience of mental health assessment across the full range of care settings including in-patients &/or Home Treatment
  • Team leading
  • Significant post registration experience.
  • To understand the need of change and demonstrate the ability of facilitating this with the Mental Health
  • To have a working knowledge of the MHA 1983
