Job overview

To ensure the mental health needs of adults with mental illness referred to Hillingdon

CMHT are appropriately assessed, supported and a high level of quality care is delivered within available resources.

To provide support and advice to users, carers and other professionals on mental health issues.

Main duties of the job

  1. Management of a CPA caseload, medication administration, Duty work and attendance to MDT meetings.
  2. To undertake holistic assessments of service users utilising the integrated notes system (SystmOne), which will incorporate the Single Assessment Process, Risk Assessment and Mental State examinations.
  3. To promote the work of the Hillingdon CMHT and an understanding of mental health issues affecting adults with mental illness.
  4. To maintain up-to-date clinical records and documentation (including electronic records) in line with agreed local policies and procedures, NMC guidelines and CNWL policy.
  5. To ensure that own professional practice and clinical knowledge is maintained at all times in accordance with NMC guidelines and CNWL policies and procedures.
  6. To work in partnership with the primary care team, statutory and voluntary agencies to ensure that the appropriate care needs are met.

We are service user-led and work with adults with mental and physical health issues who require support from a multidisciplinary approach. We have recently transformed to the new Hub model and therefore it is an exciting opportunity to join the team when services are changing to be more responsive to the service users within the Hillingdon borough.

We are a multidisciplinary service, comprising of mental health nurses, occupational therapists, psychiatrists, psychologists, social workers, employment specialists, and peer support workers. The team is supported by a good administrative system and offers an excellent work environment. The team has good links with local statutory and non-statutory services.

Detailed job description and main responsibilities

  • The Community Mental Health / Care Co-ordinator will work as a member of CMHTs in Hillingdon.
  • The post holder will assess and support a defined caseload of service users living in the community who have a range of complex mental health needs.
  • They will undertake holistic community assessments of service users referred to the team utilising the Single Assessment Process, Risk Assessment tools, Mental State examination and Integrated clinical notes system.
  • The post holder will promote an increased awareness of mental health issues for adults with mental illness within the community and work closely with primary care and statutory agencies.
  • They will provide supervision to students on fieldwork placement with the team and will contribute to the development and learning for themselves and others.

Skills/Abilities

Essential criteria
  • Ability to participate in the work of the M.D.T.and contribute to the work of the team
  • To demonstrate a knowledge of a range of therapeutic assessment and interventions available to reduce distress for individuals experiencing an acute mental health crisis or with an ongoing support need (risk assessment, MSE, capacity assessment, Full needs assessments) and manage a complex caseload effectively.
  • Ability to teach and supervise qualified and unqualified staff
  • To demonstrate their understanding of legislation
  • Ability to assess and support individuals experiencing acute mental illness distress and crisis
  • Ability to determine priorities against conflicting demands
  • Ability to reflect on practice and interpersonal relationships
  • Verbal, non-verbal communication skills and telephone skills.
  • Skills in a range of therapeutic interventions
  • Skills in psycho-social treatment options
  • Demonstrate clinical leadership

Qualification

Essential criteria
  • Evidence of post basis education
  • RMN/ OT/ SW post qualification

Experience

  • Substantial years’ experience in post registration.
  • Substantial experience of working with or within the community setting
