Swydd ddisgrifiad a phrif gyfrifoldebau manwl

Lead/facilitate robust gatekeeping 2 assessments making clear decisions, based on an analysis of complex presenting problems and judgement about available options. This will include those related to the admission and discharge of patients (when working with out of hours service providers such as home treatment teams, bed management and/or on-call SpR and AMPH’s can be utilised to provide support).
Manage the trust health based place of safety (i.e. s.136 suite), ensuring trust polices are adhered to where appropriate i.e. seclusion policy.
Utilise a wide variety of clinical skills appropriate to the needs of individual patients and the clinical setting in which they are required to be seen.
Ensure discharge from Lotus is clinically sound and timely.
Have an ability to engage challenging and/or acutely distressed patients in potentially hostile and/or antagonistic situations.
Provide a broad range of specialist psychosocial treatments for individual patients and a specialist consultation-liaison service to referring clinicians and clinical teams, assisting them in decision making in difficult and complex cases.
Work with colleagues to develop and promote the service’s philosophy, framework of care delivery and strategic objectives.
Deliver care that is evidence based, reflecting current best practice and research.
Participate in structured clinical supervision in accordance with Trust policies and procedures.
Demonstrate sound understanding of the legal and ethical issues in caring for the mentally ill. In particular, demonstrating sound understanding of the Mental Health Act /Mental Capacity Act and its use in a non-mental health inpatient setting.
Promote recovery and social inclusion throughout the patient journey.
Help the client identify strengths to achieve goals and aspirations.
Contribute to a comprehensive risk assessments in accordance with the Trust Policies and Guidelines.
Raise safeguarding alerts if you suspect that a service user and/or carer has been exposed to harm or abuse.
Demonstrate the ability to receive and give complex clinical details clearly using SBAR, with patients, carers and colleagues.
Communicate clear decisions and rationale when agreement cannot be reached e.g. in situations where the Mental Health Act or Mental Capacity Act or Common Law may be needed to be used.
Liaise with other individuals and services in the interest of collaborative crisis and discharge planning for patients, i.e. social services, community teams, carers/families, drug and alcohol teams.

Background

Every three minutes someone in the UK develops dementia: a progressive and complex condition that can be devastating for the whole family. One in two of us will be affected by dementia in our lifetime – either by caring for someone with the condition, developing it ourselves, or both.

Dementia UK is the specialist dementia nursing charity that is there for the whole family. Our specialist nurses, known as Admiral Nurses, provide free, expert advice, support and understanding to help families care for their loved one. Every day, Admiral Nurses help families up and down the country to have the best life possible, for as long as possible.

Purpose of the job/about the role

Dementia UK has entered into an exciting three-year corporate partnership with a national partner. As a key component of this initiative, we will establish face-to-face Admiral Nurse clinics across the UK. This new team will bring Admiral Nursing to high street locations across England, Scotland, Wales, and Northern Ireland.

As part of the national Clinics Programme Team, you will provide in-person biopsychosocial support and advice, sharing your specialist dementia nursing expertise to help families manage the complexities of the condition. Your focus will be on supporting family carers, people affected by dementia, and professionals who require specialist dementia guidance in areas of the UK where Admiral Nurse services are currently unavailable, bringing a much-needed service directly to local communities. Throughout your work, you will consistently promote Admiral Nursing and a family-centred approach to dementia care.

You will receive full support with your continuous professional development from the Admiral Nurse Academy.

As this unique nursing role offers you the opportunity to travel across the country, and will include overnight stays, flexibility is essential. When not delivering face-to-face clinics, you will provide support on our Helpline, working from home.

Admiral Nurse – Delivered Services

Join our Delivered Services team as an Admiral Nurse and bring your specialist dementia nursing expertise to support family carers, people affected by dementia, and professionals who require specialist advice. You’ll be a key member of our team, providing biopsychosocial support through our National Dementia Helpline and Virtual Clinic appointments.

In addition to your core responsibilities, you’ll have the opportunity to lead and support other clinical projects across the charity, benefit from monthly clinical supervision, and participate in a range of learning opportunities to support your ongoing professional development.

As an Admiral Nurse, you’ll be responsible for providing nursing support and expertise, offering practical guidance to colleagues, the general public, those with dementia, and others in supporting family and relationship-centered approaches to dementia care. You’ll also lead and coordinate shifts as required, providing clinical supervision to others and assist with the development, evaluation, audit of the service, carer and supporters in different settings.

To be successful in this position, you’ll need to be an active Registered Nurse on the NMC register with a post-registration qualification or training in dementia care or a relevant subject. You will have significant experience of practicing as an Admiral Nurse or dementia specialist nurse and previously worked with people who have dementia and their family, carer and supporters in different settings.

Strong communication skills are essential, as is a proven track record of providing high standards of clinical leadership and specialist nursing support. You should be adept at providing clinical advice, clinical supervision, and advocating for family carers.
