A fantastic and exciting opportunity has arisen for the post of Mental Health Advanced Clinical Practitioner (ACP) for Twickenham, Integrated Recovery Hub (IRH). The Advanced Clinical Practitioner will use, highly specialist knowledge and skills to provide healthcare autonomously to people within their sphere of practice. The ACP is accountable for independent clinical assessment, diagnosis and treatment of people which includes medication initiation, prescribing and monitoring. In addition to this, the MH ACP is also responsible for appropriately referring people to relevant specialties for any necessary investigations.

This role requires someone with drive, energy and a commitment to high performance standards and the delivery of person centred, evidenced based care. The post holder would be joining a supportive MDT, with supervision and opportunities to participate in service development.

The successful candidate will need to demonstrate excellent clinical skills and knowledge, and interpersonal skills, an aptitude for managing multiple demands and effective time management techniques whilst remaining calm under pressure.  A positive outlook and ability to motivate others is also desirable.

You would need to have excellent communication skills, pro-active, be able to work autonomously and in complex environments. Interested candidate would be expected to be able to think creatively, innovatively and demonstrate ability to provide senior clinical input to staff. You will also be flexible, responsive, and organised, with an interest in developing services, and participating in service modernisation.

  • To act as duty for the team on a regular basis overseeing referrals and manage the team’s response to requests for urgent assessmen
  • To promote and develop multi-disciplinary working within the team ensuring that respect is fostered between the disciplines.
  • To manage a caseload of rough sleepers with complex needs as agreed with line manager and within the service acceptance criteria. To conduct assessment, liaison, and brief intervention, ensuring that lines of communication are effective and regular reviews are in plac
  • To keep accurate up-to-date records of all contacts with service users and communications with other professionals involved in their care ensuring that electronic records are updated regularly and distributed as appropriate. 
  • To maintain and develop collaborative working relationships with service providers from a wide range of agencies
  • To develop and maintain a high standard of practice based on a comprehensive assessment using a range of psychosocial and clinical approaches.
  • To establish joint and / or co-working practice for clients with other team members and /or external service providers as appropriate.
  • To fully participate in the assessment and care of rough sleepers with mental health needs, implementing appropriate interventions with them and their carers, based on up-to-date theory, the practice model of the service and relevant other guidance
  • To undertake risk assessments with team members and health professionals from other agencies, recording and sharing information as necessary and within confidentiality guidelines, in manner which is timely to ensure the safety of the service user and others.
  • To visit rough sleepers in an environment that suits them, on the street, day centre, work, and recreational centres (as appropriate) to engage and encourage participation in treatment and in all aspects of daily living.

  • To hold a PGDip in Advanced Clinical Practice or MSc in Advanced Clinical Practice
  • Portfolio of evidence for advance clinical practice consolidation
  • Evidence of continuing professional development

  • Evidence of education/training in specialist area (i.e. therapies, interventions, service-specialism)
  • Post graduate certificate in education

Experience

  • Extensive experience of leading the delivery of care for a diverse population
  • Evidence of mental health comprehensive assessments, diagnosis, formulation and range of treatments
  • Experience working within healthcare services at an advanced level of practice
  • Advanced level understanding of specialist-specific disorders and their corresponding evidence-based interventions
  • Contemporary knowledge of relevant legislation and healthcare policies
  • Applied understanding of issues relating to confidentiality and mental capacity
  • Applied knowledge of safeguarding, inclusion and equality
  • Experience of leading research/quality improvement initiatives, developing and evaluating new ways of working
  • Experience of working in Older Peoples and/or working age adult community mental health services (For those working in an Older peoples services ACP role only)
