To act as duty for the team on a regular basis overseeing referrals and manage the team’s response to requests for urgent assessmen

To promote and develop multi-disciplinary working within the team ensuring that respect is fostered between the disciplines.

To manage a caseload of rough sleepers with complex needs as agreed with line manager and within the service acceptance criteria. To conduct assessment, liaison, and brief intervention, ensuring that lines of communication are effective and regular reviews are in plac

To keep accurate up-to-date records of all contacts with service users and communications with other professionals involved in their care ensuring that electronic records are updated regularly and distributed as appropriate.

To maintain and develop collaborative working relationships with service providers from a wide range of agencies

To develop and maintain a high standard of practice based on a comprehensive assessment using a range of psychosocial and clinical approaches.

To establish joint and / or co-working practice for clients with other team members and /or external service providers as appropriate.

To fully participate in the assessment and care of rough sleepers with mental health needs, implementing appropriate interventions with them and their carers, based on up-to-date theory, the practice model of the service and relevant other guidance

To undertake risk assessments with team members and health professionals from other agencies, recording and sharing information as necessary and within confidentiality guidelines, in manner which is timely to ensure the safety of the service user and others.

To visit rough sleepers in an environment that suits them, on the street, day centre, work, and recreational centres (as appropriate) to engage and encourage participation in treatment and in all aspects of daily living.

Experience

Extensive experience of leading the delivery of care for a diverse population
Evidence of mental health comprehensive assessments, diagnosis, formulation and range of treatments
Experience working within healthcare services at an advanced level of practice
Advanced level understanding of specialist-specific disorders and their corresponding evidence-based interventions
Contemporary knowledge of relevant legislation and healthcare policies
Applied understanding of issues relating to confidentiality and mental capacity
Applied knowledge of safeguarding, inclusion and equality
Experience of leading research/quality improvement initiatives, developing and evaluating new ways of working
Experience of working in Older Peoples and/or working age adult community mental health services (For those working in an Older peoples services ACP role only)

Background

Every three minutes someone in the UK develops dementia: a progressive and complex condition that can be devastating for the whole family. One in two of us will be affected by dementia in our lifetime – either by caring for someone with the condition, developing it ourselves, or both.

Dementia UK is the specialist dementia nursing charity that is there for the whole family. Our specialist nurses, known as Admiral Nurses, provide free, expert advice, support and understanding to help families care for their loved one. Every day, Admiral Nurses help families up and down the country to have the best life possible, for as long as possible.

Purpose of the job/about the role

Dementia UK has entered into an exciting three-year corporate partnership with a national partner. As a key component of this initiative, we will establish face-to-face Admiral Nurse clinics across the UK. This new team will bring Admiral Nursing to high street locations across England, Scotland, Wales, and Northern Ireland.

As part of the national Clinics Programme Team, you will provide in-person biopsychosocial support and advice, sharing your specialist dementia nursing expertise to help families manage the complexities of the condition. Your focus will be on supporting family carers, people affected by dementia, and professionals who require specialist dementia guidance in areas of the UK where Admiral Nurse services are currently unavailable, bringing a much-needed service directly to local communities. Throughout your work, you will consistently promote Admiral Nursing and a family-centred approach to dementia care.

You will receive full support with your continuous professional development from the Admiral Nurse Academy.

As this unique nursing role offers you the opportunity to travel across the country, and will include overnight stays, flexibility is essential. When not delivering face-to-face clinics, you will provide support on our Helpline, working from home.

Admiral Nurse – Delivered Services

Join our Delivered Services team as an Admiral Nurse and bring your specialist dementia nursing expertise to support family carers, people affected by dementia, and professionals who require specialist advice. You’ll be a key member of our team, providing biopsychosocial support through our National Dementia Helpline and Virtual Clinic appointments.

In addition to your core responsibilities, you’ll have the opportunity to lead and support other clinical projects across the charity, benefit from monthly clinical supervision, and participate in a range of learning opportunities to support your ongoing professional development.

As an Admiral Nurse, you’ll be responsible for providing nursing support and expertise, offering practical guidance to colleagues, the general public, those with dementia, and others in supporting family and relationship-centered approaches to dementia care. You’ll also lead and coordinate shifts as required, providing clinical supervision to others and assist with the development, evaluation, audit of the service, carer and supporters in different settings.

To be successful in this position, you’ll need to be an active Registered Nurse on the NMC register with a post-registration qualification or training in dementia care or a relevant subject. You will have significant experience of practicing as an Admiral Nurse or dementia specialist nurse and previously worked with people who have dementia and their family, carer and supporters in different settings.

Strong communication skills are essential, as is a proven track record of providing high standards of clinical leadership and specialist nursing support. You should be adept at providing clinical advice, clinical supervision, and advocating for family carers.
