Join our dedicated team in a Nurse-led Clozapine clinic, where you'll play a key role in providing holistic care to patients with complex needs. This position involves:
- Physical Health Support: Conducting physical health screenings and assessments for Clozapine patients, with weekly support from a Pharmacist and independent practice on other days.
- Phlebotomy Support: Assisting with blood sampling as needed to ensure timely monitoring and care.
- Care Planning: Collaborating with experienced clinicians to design, prepare, and review comprehensive Clozapine and physical health care plans tailored to each individual's recovery journey.
- Community Engagement: Encouraging patients to connect with their local communities, offering both practical and emotional support.
- Comprehensive Care Coordination: Participating in physical health assessments, safety planning, and multi-agency discussions to ensure seamless and effective care.

Main duties of the job
The Health Care facilitator/Phlebotomist will work closely with the Nurse Led clinic team to support them in achieving the key performance indicators of Physical health screening and bleeding patients in timely manner to ensure patient care and needs are met. You will support clinicians and pharmacist in designing and delivering collaborative Clozapine care plans with the individuals we support following physical health screening.
This may often include encouraging the individuals ’s well-being through a planned program of personal, practical and emotional support and participation in activities of daily living, engagement in their local communities and support with understanding available treatment.

Will require to attending patients for screening and assessment for Physical Health in their home environment when they are unable to attend the clinic
Will require travelling and supporting with the Clozapine clinics across, Hounslow, Ealing and Hammersmith and Fulham
Having a car and driving licence is essential for this position

Qualifications and Training
Essential criteria
- Willing to undertake training identified by the manager as relevant to Clozapine CARMHS working.
- Good standard of education to GCSE standard or equivalent
- Phlebotomist trained and have experience working in a busy blood clinic
Desirable criteria
- Health and Social Care qualification.
Experience
Essential criteria
- Experience of mental health needs and the impact on individuals and family.
- Experience working in a Nurse led Clinic environment
- Able to support Clozapine nurse to order medication and storing it safely in treatment room
Desirable criteria
- Experience of working with vulnerable individuals.
- Have worked in busy Venepuncture clinics
- Trained to use POCHI machine for Clozapine analysis
Knowledge
Essential criteria
- Demonstrate a sound knowledge of the Community Mental Health Framework and the NHS Long term plan.
- Confidence and ability to work with individuals in their homes.
- Good written and verbal communication skills.
- Ability to contribute in the planning, implementation and review of collaborative care-plans.
- Experience of supporting people with activities of daily living.
- Experience in supporting access to community activities and support networks.
- Demonstrate an ability to be aware of diversity.
- IT literate.
- Able to complete tasks which relates to their role in supporting Clozapine nurses
Desirable criteria
- Knowledge of another language that reflects the local community would be an asset.
- Able to support Nurse to in running the POCHI machine to analyse blood sampling

- Ability to work as part of a multi-disciplinary team
- Candidate is able to demonstrate that they are able to show empathy towards individuals with a mental health needs as well as show a positive attitude towards mental health care.
