Job overview

The Community Children’s nursing team is part of an integrated network of services for children in Bexley and Greenwich providing high quality specialist child centred care. The team of specialist and children’s community nurses provides highly skilled, nursing care to children and families with nursing needs in their own homes or community settings.

To provide a high standard of evidenced based nursing care for children and young people with the following conditions: acute and long term conditions, children with disabilities and complex conditions including those requiring palliative and end of life care. 

This is a post suitable for a Children’s  Nurse who should have an interest in or a willingness to develop and extend specialised skills and knowledge within a community setting.

Evening and weekend shifts will be required on a pro rata basis.

Main duties of the job

  • Ensuring the Community Children’s Nursing and Hospital at Home Service (0-19 years) provides a high quality, evidence based locally appropriate service to families.
  • To work as part of the team providing nursing assessments and interventions to children.
  • To work in partnership with children and families to implement individualised care plans to meet medical and nursing needs.
  • To be responsible for assessing, planning, implementing and evaluating individual children’s care plans.
  • To refer children to acute and other services as appropriate and required.
  • To work collaboratively with other agencies to deliver clinical care pathways for children and families.
  • To work as part of the Community Children’s Nursing Team to provide support in an  area of special interest from acute to community settings
  • To provide support with specialist training to meet the child’s needs. This may be to the child, their family or other professional working with the child in a variety of community settings.
  • To attend specialist clinics with families to work towards multi professional assessment and care planning with the child and family.  To work as part of a Multidisciplinary team to ensure a smooth discharge from acute care to community settings as well as smooth transitions into Adult Services
  • To participate in CAF/TAC, becoming a key worker for families where appropriate

Detailed job description and main responsibilities

You will work within an integrated Children's Nursing team covering both CCN & H@H, the role as a community children's nurse is highly varied . We are one large team covering both borough's of Greenwich & Bexley. The caseload is varied with referrals of CYP who have complex medical needs, long term health conditions and short term health conditions requiring rapid service input to enable CYP to be discharged from hospital early.

Education

Essential criteria
  • RSCN or RN (child) or Learning Disability Diploma / degree level.
  • Current NMC registration
Desirable criteria
  • Evidence of continued professional development

Experience

Essential criteria
  • Clinical skills in paediatric nursing
  • Experience in assessing children’s care needs, planning implementation and evaluation of care plans.
Desirable criteria
  • Experience of joint working together with other professionals.

Skills/Abilities/Knowledge

  • Excellent communication skills and interpersonal skills
