Senior Cardiac Rehabilitation Nurse - Band 7

Full time - 37.5 hours per week

Site: The Centre, Manor Park

This is an exciting opportunity for an experienced nurse with an interest in heart failure to join the cardiac rehabilitation team in Newham.

Newham is a rich and vibrant community with a diverse population where you will have the opportunity to support patients, their families and carers through their cardiac rehabilitation journey.

You will be an RGN with post registration experience in an acute cardiac specialty including the care of patients living with heart failure and have successfully completed the cardiology pathway or ENB 124 or ENB 254 equivalent.  You will have excellent communication skills and be able to work effectively as part of a team and independently,

Main duties of the job

To assess, plan, implement and evaluate care for a caseload of cardiac rehabilitation patients living with heart failure and make referrals to the multidisciplinary team as indicated, leading by example as an expert practitioner.  You will be liaising closely with the local Heart Failure service and there will be the opportunity for increasing your skills and knowledge in this specialty.

Detailed job description and main responsibilities

  • To process and manage referrals appropriately within designated waiting times and this is delegated to the multidisciplinary team effectively and efficiently.
  • To undertake interventions consistent with evidence-based practice, transferring and applying knowledge and skills to meet patients needs.
  • To use skills and knowledge to make referrals for diagnostic tests and instigate investigations in conjunction with the patients GP or consultant.
  • To liaise with the Newham Hospital (Barts Health) Heart Failure Service and attend relevant Multidisciplinary Team (MDT) meetings.
  • To be aware of vulnerability within the patient/carer caseload and report potential safeguarding situations according to local policy.
  • To collaborate with patients and their carers to enable them to make informed decisions regarding their health and healthcare.
  • To support patients, their carers and relatives by providing up to date information on secondary prevention (which meet British Association of Cardiovascular Rehabilitation and Prevention Standards).
  • To utilise a variety of strategies to communicate effectively with patients and those involved in their care.
  • To evaluate outcomes of screening procedures, and be able to interpret clinical results and review patient holistically and take action as appropriate to the patients needs. patients and those involved in their care.
  • To provide a seamless services between primary and secondary care.
  • To support the Team in running the Education and Exercises classes across site in the locality.
  • To run Secondary Prevention clinics in the community.
  • To do home visits as part of the service specs. 

Person specification

Education/ Qualification/ Training

Essential criteria
  • Registered Nurse
  • Specialist post registration nursing course
  • Possession of relevant degree or equivalent experience
  • ENB 997/998, Mentorship course or equivalent
Desirable criteria
  • Health promotion course
  • BACPR training

Experience

Essential criteria
  • Significant clinical experience of nursing acute cardiac patients
  • Clinical Experience of nursing patients with cardiac conditions including heart failure
  • Evidence of continual professional development
Desirable criteria
  • Evidence of clinical leadership qualities
  • Cardiac rehabilitation experience

Knowledge and Skills

Essential criteria
  • Management and organisational skills including innovation to drive the service forward.
  • Excellent communication skills
  • Willingness to increase scope of practice through education.
  • Ability to work autonomously and as a team member
  • Ability to conduct and evaluate own projects successfully
  • Computer literacy accessing e-mails, Microsoft word, excel etc.
  • Ability to identify patient & staff diversity issues & proactive addressing of these in service provision
  • Significant experience of cardiology/cardiothoracic nursing
  • Up to date knowledge of heart failure treatment and medications
