If you are excited about developing and implementing services to address health inequalities, then this is an exciting opportunity for you to join Whittington Health’s community heart failure service. This is also an ideal secondment opportunity for those with acute cardiac experience to develop there skills and/or those with community heart failure experience.
This involves identification, management, and interventions for adults at risk of developing/with LTCs in Haringey’s neighbourhoods. A focus on heart failure, plus voluntary sector support to work with people at risk of/with LTC conditions and multi-morbidity, is particularly relevant to underlying need, and associated with high admissions/complications, in these communities.

The heart failure nurse will help provide a high-quality specialist nursing service for heart failure (HF), with the primary focus in delivering the health inequality work in Haringey but working alongside the existing heart failure team to residents in Islington and Haringey.
You will manage a patient caseload and promote excellence in clinical nursing skills through the provision of expertise in HF to peers and community carers, working closely with other members of the community HF team, the relevant cardiologists and Whittington Health / North Middlesex University Hospital Consultant HF leads, and general practitioners.
You will also facilitate the implementation of agreed HF care to patients, through close collaboration with the CCGs, the extended HF team, general practice, and Whittington Health HF Lead.
This post is available due to the expansion of the well-established heart failure nurse team.

These patients will need support with both physical and mental health wellbeing. There is an emphasis on delivering patient education to enable self-management e.g. about their condition and this may be face to face and/or virtually (telephone or video) or via telehealth. The role will involve liaising with different health care professionals and a range of outside agencies. You will need excellent communication skills, be able to work independently and be motivated to deliver the patient’s goals to a high standard.

- NVQ Level 2 or equivalent standard of literacy and numeracy. • Registered General Nurse to degree level or equivalent • Post registration training or equivalent in coronary heart disease management. Post registration training or equivalent experience in community services.

- Advanced physical assessment course or commitment to attain if post becomes substantive • Nurse Prescriber or commitment to attain if post becomes substantive

- Basic awareness of IT and IT Skills- Microsoft word, Power Point, Excel, RIO. • Correct interpretation or complex cardiac investigations • Excellent communication skills • Ability to work as part of a team and use own initiative • Ability to manage time effectively, meet deadlines and work under pressure.

- Demonstrate Audit and Evaluation • Experience of delivering structured education and training to large group and one to one
Knowledge and Experience

- An awareness of NHS Plan, NSF and Clinic Governance priorities • Evidence of further study in relation to cardiac disease • Understanding of the palliative care agenda • Minimum 2 years experience of caring for patients with cardiac condition • Evidence of post registration development and continues learning • Experience of managing complex situation
