Job overview
Hounslow Health Primary Care Network is committed to providing excellent patient centred care for approximately 90,000 residents in Hounslow borough.
We are looking for an advance clinical practitioner nurse (ACPN) to join our PCN to deliver high quality and effective healthcare to our patients. You will be dynamic, enthusiastic, self-motivated, highly experienced nurse with excellent clinical general practice & knowledge. In this role you will use your skills as an advanced practitioner in diagnostics and therapeutics, enhanced level consultation, critical thinking with clinical decision-making, and leadership.
Candidate should ideally be practising as an ACPN.

- Scope of practice in assessment and triaging is desirable
- The ability to thrive when working in a devolved, flexible, autonomous environment
Main duties of the job
You will work as part of a multidisciplinary team to:
- Work with practices within the PCN to develop effective, evidence-based care for the PCN's population.
- Contribute to the development of joint standards of care, clinical guidelines and management pathways for utilisation within practices
- Develop good relationships between local community services, secondary care and the teams within the PCN
- Be part of the PCN team providing peer support along with education and training
