An exciting opportunity has arisen to become an enhanced nurse specialist as part of a heart failure specialist team. This is a one year fixed term post. We provide care in both inpatient and outpatient settings and are involved from first diagnosis to end stage heart failure. The role will be focused on ensuring evidence-based medicines are optimised, providing lifestyle and self-care advice to patients and carers, and ensuring the patient has a comprehensive management plan. We hold weekly multidisciplinary meetings where we can discuss complex patients and access the expertise of our cardiac rhythm management, consultant colleagues, advanced heart failure and palliative care experts. We are excited to be part of new and inventive ways of working through using information technology and services such as virtual wards to support patient care which is a key duty involved in this role. The post holder will be a key member of the team influencing service development and involved in audit and research.

Prif ddyletswyddau'r swydd

The main duties of this post will be to to facilitate the provision of high quality specialist care to patients with acute and chronic heart failure. This will involve assessing the individual needs of patients using assessment skills and utilising various investigations. The post holder will need to recommend heart failure medications to improve quality of life and reduce morbidity and mortality. They will be expected to promote self management and provide health education.

A  key responsibility will be to supporting the management of patients on the virtual ward platform. Virtual wards support patients who would otherwise be in hospital to receive the acute care, monitoring and treatment remotely in their own home. This includes either preventing avoidable admissions into hospital, or supporting early discharge out of hospital. The role will involve identifying appropriate patients for the virtual ward pathway and supporting the use of remote monitoring.

Experience

  • Minimum 5 years recent cardiology experience with minimum 2 years at band 6 in cardiac environment
  • Evidence of teaching and developing others
  • Ability to analyse and interpret ECG’s, Echos and routine blood tests
  • Extensive experience of nursing patients living with heart failure.
  • Experience of service planning/managing change e.g. provide examples of projects/research undertaken under own initiative
  • Experience of running nurse led clinics
  • Experience of managing own caseload of patients
  • Experience of the provision of cardiac rehabilitation and secondary prevention
