Job overview

The Barnet Children’s Continuing Care Team is a small, friendly, dedicated team of nurses who oversee and case manage the processes of delivering Continuing Health Care required for children and young people in Barnet with complex health needs. The children and young people eligible for Continuing Health Care are identified in accordance with the National Framework for Children and Young People's Continuing Care 2016.

Our children, young people and their families are at the heart of everything we do. The post holder will work within the team and with partner agencies across Barnet to ensure that commissioned packages of care is of high quality, safely delivered and based on individualised needs.

Main duties of the job

The post holder will be part of the Barnet Children’s Continuing Care Team and, in a role, ideally positioned to make a positive difference to the lives of children and young people with complex health needs. The team is committed to developing its staff and opportunities are available to enhance your professional development.

Essential criteria
  • Any of the following: RGN, RSCN, RN (Child) RN (LD) NMC Registration Evidence of post registration education
Desirable criteria
  • Teaching qualification

Experience

  • Relevant nursing experience
  • Experience of liaising / negotiating with professionals
  • Experience of teaching/supporting families with their care package
Desirable criteria
  • Case management
  • Experience of working in culturally diverse communities

Skills & Knowledge

  • Expertise in nursing assessment / clinical skills
  • Ability to work independently Time management and prioritisation skills
  • Excellent communication skills, both verbal and written
