Job overview

Are you a Band 5 Nurse who would like to join our dynamic team? We are seeking an ambitious, motivated, compassionate and experienced Band 5 nurse to support the changing face of community health care. As part of the Urgent Community Response and Virtual Ward initiative, your
role will be essential in providing efficient, integrated, rapid care to our patients with care and respect.


You will be working as part of the Primary Care at Home team predominantly on Unplanned Care and be able to assess, plan, implement and evaluate patient care, allocate work and complete onward referral when required. There is an expectation that you would will have the ability to be flexible to cross cover into planned care if required.


You will be working closely with other members of the MDT and partners, alongside senior clinical staff within your team to formulate a management plan that will support the patient to remain in the community.

Main duties of the job

Experience in the field of adult community services is desirable but a willingness to learn and contribute is more important. Consideration will also be given to staff who are newly qualified and are passionate about a career in community nursing. A flexible approach is required to accommodate the changing needs of the service along with the drive to influence service development.

You will have regular supervision, mentorship and support from within the Primary Care at Home Team and the Trust.


You must have a driving license and have access to a vehicle in order to carry out your duties. You will be required to work a shift pattern covering 7 days a week from 08:00-20:00.

 To anticipate barriers to communication and use interpreters or
tools as required.
 To use persuasive skills of negotiation and problem
solving to facilitatepositive outcomes.
 To provide education to patients, students and colleagues as
required.
 To produce accurate and complete records consistent
with legislation, policy and procedure.
 To attend and participate in relevant meetings and training events.
 To build and develop relationships between health
professionals involved in the patient pathway.
 Provide understanding and support to patients coping with
acute and/or long-term conditions, including promotion of selfcare.

Essential criteria
  • Current UK driving license and access to a car
  • Ability to travel across sites and across Trust boundaries to meet patient needs and attend meetings, etc.

Eduacation

Essential criteria
  • Registered General Nurse
  • Demonstrate evidence-based practice and up-to-date clinical skills.
Desirable criteria
  • Evidence of post-basic professional development
  • Relevant teaching/mentorship qualification or commitment to undertake
  • Post-registration nursing experience

Experience

Essential criteria
  • Evidence of and commitment to multi- professional working
  • Understanding of the interface between primary and secondary care
  • Comprehensive understanding of the roles of others in health and social care

Knowledge and skills

Essential criteria
  • Literate in IT/Computer Skills
  • Evidence of knowledge of clinical governance
  • Knowledge of the principles of Safeguarding of Vulnerable Adults and Children
  • Knowledge of the principles of health promotion and prevention of illness
  • Good oral communication skills based on fluency in English language
Desirable criteria
  • Demonstrate an understanding of national government initiatives in health care.

Physical/Mental and Emotional Effort

Essential criteria
  • Ability to mobilise and stands for most of shift; and kneels and crouches to deliver clinical care e.g. dress wounds
  • Ability to concentrate on providing clinical care such as drug calculations, carrying out tests and managing interruptions and unpredictable patient/client /carer behaviour
  • Occasional distressing or emotional circumstances such as imparting unwelcome news, care of terminally ill or safeguarding issues
  • Post holder could work in occasional unpleasant conditions during home visits and could come across unpleasant body odours, dust, noise/ Body fluid or even verbal aggressions
