Job overview

To provide and manage expert specialist clinical practice in the borough of Croydon for community/housebound people with diabetes.

To be a source of expertise, knowledge and support, upskilling community health care professionals through joint and virtual clinics.

To develop evidence based clinical nursing in line with current research and guidelines.  To ensure people with diabetes and their families are fully informed and supported in the management of their diabetes care. 

To work with the wider multidisciplinary team in a proactive way with the aim of preventing hospital  admission, supporting early discharge and reduced GP contact. 

To work within the community based services to provide structured education for people with diabetes.

To provide nurse led clinics and joint clinic in community locations to support the management of people with diabetes.  This will include initiating diabetes medication and insulin as well as monitoring people ongoing glycaemic control and titrate/optimise medication accordingly.

Full time position (37.5 hours)

Our Integrated Diabetes Service provides seamless, high quality care for the people of Croydon. 

You will provide clinical / education support for the Primary Care Team, people with both type 1 and type 2 diabetes and their carers.   

We offer education/support to help with personal development, Apprenticeship offerings and other new development opportunities. 

You will have the opportunity to be part of the specialist services which include:

  • Antenatal  care - Diabetes in pregnancy clinics
  • Renal clinics
  • Transition and young persons’ clinics
  • Insulin pump clinics 
  • Continuous Glucose Monitoring (CGM) & Flash glucose monitoring clinics and other Diabetes technologies
  • Joint clinics with Psychologist & or Dietitian and other Health care Professionals including GPs

We are seeking to recruit Diabetes Specialist Nurse who have a passion for improving the care of people with diabetes, excellent communication skills and enjoy working as part of a team. 

The role will include working closely with colleagues in hospital, community and primary care to ensure that the people of Croydon receive high quality integrated care. 

A clean driving license and use of a car for work purposes is desirable.

  • Extensive experience in Adult diabetes nursing, including working as a Adult diabetes specialist nurse
  • Able to provide support, education and specialist input for the management of more complex patients and improved management of diabetes.
  • Acts as a link between the patient and other healthcare professionals and provides patient-centred care that meets the physical, psychological, social and spiritual needs of the patients and their families.
  • Is a highly skilled Independent practitioner in diabetes care who is able to motivate, counsel, support, educate, coordinate and help manage the care of the people with diabetes.
  • Is competent to educate other healthcare professionals both community and in hospital settings, families, and young adults. Such education involves discussing any concerns, misconceptions or beliefs the patient or family may have, as well as offering advice on diet and lifestyle.
  • Has the ability to focus on early detection of diabetes and is able to intervene and manage the condition early on.
