Clinical Team Leader -Community

An exciting  opportunity has arisen for a team leader in Dulwich Community Mental Health Team. We are looking for a highly experienced and enthusiastic senior clinician who has a real passion for mental health and who promotes the highest standards of care in their practice.

The post holder will lead a community based multidisciplinary mental health team that provides on-going holistic assessments, treatment and support to service users with diagnosis of a severe mental illness. Crucially enabling staff to provide a high quality service for service users and carers.

You will need to be able to provide effective managerial and clinical leadership in the team and will coordinate and lead on internal and external referrals.

In exchange, you will be supported through regular supervision and appraisal. We are committed to helping to develop the people that come to work with us. 

 

Prif ddyletswyddau'r swydd

The post-holder will:

·       Have overall management responsibility for the delivery of care within the Dulwich Community mental health team, in accordance with Trust policies and procedures.

·       Lead a team of professional and support staff in the delivery of high quality evidence-based practice, ensuring that regular supervision and appraisal take place.

·       Manage administrative and operational resources in a cost-effective manner, evaluating the standard of service delivered.

·       Co-ordinate multidisciplinary working, maintaining excellent communication and working relationships with all team members, and other services within the Trust, to ensure effective interface and continuity of care between disciplines and services.

·       Ensure that all referrals to the service are screened, that, where necessary, service users are provided with a full assessment of health and social needs and that aftercare is provided from the most appropriate service.

·       Carry out clinical and managerial procedures to a standard that ensures safe and effective care, and complies with the relevant Code of Professional Conduct. There is an expectation that the post holder will place the needs of service users at the centre of care delivery.

About the location:

Our Trust headquarters is located at Denmark Hill less than 5 minutes from the train station (zone 2).  We also provide services and operate across other locations, such as London boroughs of Croydon, Lambeth, Lewisham and Southwark; and substance misuse services for residents of Bexley, Lambeth, Greenwich and Wandsworth.

This post will be based at Marina House, which is in Camberwell, Southwark. The nearest underground station is  Denmark Hill. Limited parking is available.

  • Ensure that all clinical practice is developed using up to date evidence base, ensuring that all staff have the appropriate skill base and supporting mechanisms to access training to work with the service user group.
  • Oversee a service user's care pathway, utilising available resources and Patient Journey framework and lead the team in its implementation.
  • Develop and maintain procedures that result in good communication between the multidisciplinary team and other agencies.
  • Deputising for the Clinical Service Lead as required
  • Ensuring that systems are in place which invite service users and their carers to comment on the quality of services they receive.
  • Implementation of the complaints procedure including investigation, response, and resolution.
  • Ensuring that their line manager is kept informed of all developments and changes within the team.
  • The creation and maintenance of an environment which is conducive to learning and which meets the educational standards required by Educational Institutions whose students are placed on the ward. The post-holder will be expected to facilitate actively the team’s involvement in the training and education of staff from a variety of disciplines. 
  • Participating in projects within the team and Trust as required by the senior management of the Directorate.

Qualifications

  • Registered Mental Health Nurse/Social Worker/Approved Mental Health Professional/Registered Occupational Therapist
  • Post-graduate diploma / specialist training or equivalent experience in working with adults with mental health problems
  • Mentorship Course (or other relevant teaching/assessment course). (A)
  • Evidence of continuing professional development (A)

  • MSc level qualification associated with adult mental health, e.g. clinical psychology, psychotherapy or equivalent, with a strong portfolio of post-registration clinical professional development.

Experience

  • At least 2 years experience, at band 6 or equivalent, using a range of clinical / managerial skills within mental health services
  • Knowledge and understanding of the Recovery model of care
  • Experience of managing staff and providing supervision
  • Experience of working with diversity, including with people from a variety of cultural backgrounds
  • Experience of participating in and managing change and overseeing ongoing service development

  • Knowledge of regional and national health and social care frameworks which are applicable to the role

Knowledge

  • A working knowledge of all relevant mental health legislation including: Mental Health Act, Care Act 2014
  • Knowledge and understanding of current issues in multidisciplinary practice
  • An understanding of the principles of Clinical Governance
  • Knowledge of the processes necessary for effective risk assessment and management
  • An understanding of the models of assessment as well as evidence based psycho-social interventions
