Band 7  Community Diabetes Nurse 

An exciting opportunity has arisen for a Band 7 Community Diabetes Nurse to work with our Integrated Diabetes Nursing service covering Basildon, Billericay, Brentwood, Wickford and Thurrock areas.

We are looking for an enthusiastic and motivated individual, with excellent communication skills,  who is able to work autonomously, as part of the team and to be an integral member of the wider Diabetes teams in supporting people managing the complexities of diabetes management.

This role requires facilitating patient structured education , having a sound knowledge of the principles of diabetes management and care, and  a keen interest in professional development & advancing with technology. Therefore experience of teaching and good IT skills is essential.

The ability to travel effectively across the boroughs is necessary.

The post holder will be responsible for providing; supporting and developing the delivery of high quality; evidence based Diabetes clinical practice throughout the local community, in health centre clinics and patients’ homes.  The successful applicant will be  required to work closely with the lead Diabetes consultant, GPs, nursing colleagues and other members of the multidisciplinary team.

The post holder will be expected to support their team, department and organisation to achieve the Trust’s Values in their day to day work.  These are the 5P’s:

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.

Probationary Period
This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High Cost Area Supplement - Fringe

This post also attracts payment for High Cost Area Supplement of 5% of the basic salary (with a minimum of £1,258 to a maximum of £2,122 p.a. pro rata for part time).

Key Responsibilities: 

To act as an autonomous, registered practitioner who is legally and professionally accountable for own unsupervised actions guided by the professional code of conduct and Trust guidelines and protocols.

  1. To be responsible for organising and prioritising own and others workload in the day to day allocation of work.
  2. To deputise when required in the team managers absence and delegate appropriately to health care assistants.
  3. To have organisational knowledge relating to Trust protocols and procedures and adhere to them, particularly administration of medicine and moving and handling.
  4. To be responsible for providing accurate records of information required by the Trust for audit purposes.
  5. To ensure effective risk management at team level by accident/incident reporting, assessing and controlling risk and ensuring residual risks are added to the Trust register.

  • Evidence of a range of CPD activities with relevance to diabetes & up-to date mandatory training

  • Non-Medical prescriber or working towards

Experience

  • Experience of post registration experience working directly with diabetes patients of at least 2 years
  • Experience of working as member of a multi-disciplinary team
  • Experience of mentoring & supervising junior staff

Knowledge

  • Up to date Knowledge and strategies of all aspects of Diabetes care, Nursing care & NICE guidelines
  • Clinical knowledge & experience of Diabetes Medication, Action and application of these medications
  • Is able to demonstrate an understanding and empathy relating to the condition of Diabetes and the challenges facing this client group

Skills
