Are you a qualified Mental Health Nurse passionate about working with older people?  Are you looking for an opportunity to work in the community in a friendly and welcoming team, rich in knowledge and experience?  Then this is the ideal position for you!

The post holder will hold a caseload and work within the principles of the agreed model of care for the service as laid down in the operational policy and will work as an autonomous practitioner within an integrated Multi-disciplinary team.

This will involve the assessment planning, implementation and evaluation of the care provided and will liaise with all agencies involved with the service user.

The post holder will be confident and competent in administering IM injections and fully aware of the Medicines Management Guidance & associated policies.

Community experience is desirable but not essential for the right candidate.

  • Undertake specialist and comprehensive assessment of needs
  • Co-ordinate care for a defined caseload
  • Undertake physical health assessments and provide or support access to appropriate intervention.
  • Practice within professional guidelines and promote professional practice in clinical area.
  • Ensure high standard and accurate verbal/written communication about patient care with all team members.
  • Put service users at the centre of everything achieved, taking responsibility for the development and delivery of evidence based practice, ensuring that it supports and reflects service user individual needs, Trust objectives and professional guidance.
  • Work to values that include self-determination, empowerment and choice for service users.
  • Work in partnership with service users and empower them to draw on their individual strengths and assets, and set goals they would like to achieve in all aspects of their lives.

To implement and review individual personal recovery plans using the services’ initial assessment/review tool and specialized structured assessment tools, under the direction of Senior Nurses.

To be the named key-worker managing the recovery plans for a named caseload for Non CPA service users

To independently administer & monitor DEPOT injections under the direction of a senior nurse and as prescribed by a registered prescriber.

To ensure service users physical, psychological, mental health and social needs are factored into assessment and recovery plans.

To empower service users by promoting; hope; positive risk taking and the sense of agency to encourage self-management, self-care, personal development and life change.

To provide therapeutic individual interventions to support personal recovery and social inclusion.

To encourage service users to make and re-connect relationships with family, friends and peers as part of the process of promoting social inclusion.

To help service users make sense of their experience of mental ill health and to normalize the expression of feeling.

To work with and support carers, including involving them in the service users care, signposting to community support networks.

To participate in Team Duty Rota.

To actively collaborate with other teams/ services and partner agencies in shared work to support recovery plans.

Knowledge

  • Able to confidently administer medication including independent depot medication administration
  • Good verbal and written communication skills
  • Knowledge of the legal requirements of the Mental Health Act and Mental Health Act

  • Knowledge of structured psycho-therapeutic interventions
  • Familiarity with or experience of inputting clinical information onto RiO

Experience

  • Qualified Mental Health Nurse currently registered with NMC having completed a preceptorship
  • Experience of working with people with mental health needs
  • Experience of working with service users and their families and carers

  • Experience of working with Older Adults with mental health needs
  • Experience of working in a health or social care community setting
  • Experience of participating in a Team Duty Rota
