Job overview

The post holder will hold a caseload and work within the principles of the agreed model of care for the service as laid down in the operational policy. The post holder will work as an autonomous practitioner within an integrated Multi-disciplinary team.
This will involve the assessment planning, implementation and evaluation of the care provided and will liaise with all agencies involved with the client. The post holder will be confident and competent in administering IM injections and fully aware of the Medicines Management Guidance & associated policies.

Main duties of the job

  • Undertake specialist and comprehensive assessment of needs
  • Co-ordinate care for a defined caseload
  • Undertake physical health assessments and provide or support access to appropriate intervention.
  • Practice within professional guidelines and promote professional practice in clinical area.
  • Ensure high standard and accurate verbal/written communication about patient care with all team members.
  • Put service users at the centre of everything achieved, taking responsibility for the development and delivery of evidence based practice, ensuring that it supports and reflects service user individual needs, Trust objectives and professional guidance.
  • Work to values that include self-determination, empowerment and choice for service users.
  • Work in partnership with service users and empower them to draw on their individual strengths and assets, and set goals they would like to achieve in all aspects of their lives.

Detailed job description and main responsibilities

  • To be responsible for an identified community caseload, ensuring high standard of professional conduct to comply with best practice and professional accountabilities.
  • To promote a service user focused service.
  • Participate in audits and survey relevant to the team.
  • To participate in development of new policies and procedures as required.
  • Ensure that clients receive a culturally competent service.
  • To co-ordinate the day-to-day activity and manage the workload of the Team.
  • To ensure the legal requirements of the Mental Health Act (1983) and subsequent amendments are adhered to
  • To ensure the legal requirements of the Care Act 2014 other legislation
  • To establish and maintain good relationships with service users, carers and voluntary sector colleagues.
  • To provide best evidence practice to service users which complies with legislation, policies and procedures and relevant quality, governance and local and national guidelines.
  • Liaise with General Practice and Primary Care colleagues to promote partnership working.
  • Ensuring that the local demographics are taken into consideration for service provision and improvement

Qualifications

Essential criteria
  • Qualified Mental Health Nurse Registered with the NMC or/ and Qualified Learning Disability Nurse
  • Evidence of Continuous professional development
Desirable criteria
  • Qualified Mental Health Nurse
  • Educated to Degree Level
  • Mentor

Experience

Essential criteria
  • Post qualification experience in Mental Health Services
Desirable criteria
  • Post qualification experience working with and supporting Older People
  • Experience of working within an NHS/Local Authority environment
  • Experience of working in a Community Mental Health Team

Knowledge

Essential criteria
  • Working knowledge of the Mental Health Act, the Care Act, the Mental Capacity Act and other social care and mental health legislation and guidance
  • Good assessment skills and ability to produce risk assessments and compile care plans in collaboration with service users and carers
  • Good IT skills with experience of working with service user data systems (ie RiO)
  • Highly developed organisational skills with the ability to prioritise your own workload
  • Ability to work both independently and as part of a team
  • provision where appropriate.  Assessment skills and the ability to compile care plans in partnership with service users, carers and others
  • Assertiveness and engagement with other professionals, able to make reasoned arguments and represent own professional views and perspectives.
  • Decision -making and problem solving, able to adopt creative and flexible approaches to meet the needs of service users
  • Highly developed organisational skills, able to prioritise workload and delegate effectively
  • Well -developed communication skills, able to communicate effectively with a wide range of audiences
  • Mental Health Practice Skills and an understanding of CPA and the NSF standards.
  • IT skills, experience of working with service user data systems
