Trosolwg o'r swydd
An exciting opportunity has arisen to become an enhanced nurse specialist as part of a heart failure team. We are a close-knit team supported by senior consultants and other members of our MDT.
We provide both inpatient and outpatient care and are involved from first diagnosis to end stage heart failure. The role focuses on ensuring evidence-based medicines are optimised, providing lifestyle advice, and ensuring patients have a comprehensive management plan. We hold weekly multidisciplinary meetings to discuss complex patients and access the expertise of, cardiac rhythm management, advanced heart failure and palliative care experts. We are excited to be part of new and inventive ways of working using IT and services such as virtual wards to support patient care. The post holder will be a key member of the team influencing service development and involved in internal audit, research, and inputting data into the national heart failure database (NICOR).
There are opportunities to continue professional development and to grow skills in enhanced practice as an autonomous practitioner and be involved in research and training and education. You will receive a comprehensive induction programme closely supported by senior nurses, and follow the BSH competency framework to guide development. There will be the possibility to progress your leadership skills through training programmes within the trust. As a trust we support flexible working patterns that meet the needs of the service.
