Job overview

This post holder will provide clinical and managerial leadership for the community Discharge to Assess beds across Bedfordshire and Luton.  You will support patients being discharged on pathways 2 and 3 by managing flow and discharge processes.  You will work alongside senior system leaders,  community therapy teams, social workers,  hospital teams, care home staff and mental health services in order to  support the pathways.

To provide senior clinical and  operational  leadership to the community beds team.  to support flow through the community beds.

Detailed job description and main responsibilities

To be part of the wider community health services senior team   providing system support at escalation meetings and on call cover as required   

To work with senior system partners to support the development of Pathway 2 and 3 procedure and process to ensure that the pathways are efficient and support a positive patient experience

You will be responsible for producing and reviewing service activity and data to understand service pressures and gaps in provision.

Experience

Essential criteria
  • Educated to masters level or equivalent level of experience of working at a senior level in specialist area
  • Extensive knowledge of specialist areas, acquired through post graduate diploma or equivalent experience or training plus further specialist knowledge or experience to master’s level equivalent
Desirable criteria
  • To have an understanding of the background to and aims of current healthcare policy in BLMK and appreciate the implications of this on engagement

Professional Registration

Essential criteria
  • A professional qualification at degree level / registration in adult nursing

Organisational Skills

Essential criteria
  • Ability to manage competing priorities in a busy and changing environment
Desirable criteria
  • Project management skills

Analytical

Essential criteria
  • Problem solving skills and ability to respond to sudden unexpected demands

Management Skills

Essential criteria
  • Must be able to prioritise own work effectively and be able to direct activities of others. Experience of managing and motivating a team and reviewing performance of the individuals.
