Senior Acute Care Nurse

This is an exciting opportunity for a qualified Mental Health Nurse to work in our Older Adult Home Treatment Team. The team provides a swift response to people who are in the acute phase of mental illness, providing home treatment in order to prevent hospitalisation. To be successful in this role, we want to hear from Practitioners who really want to ‘make a difference’, with the Trust values at the core of patient care, who have previous experience working with acute mental illness, or who are seeking to develop a career within acute care. You will need to demonstrate a commitment to providing a high-quality standard of care, excellent communication, interpersonal, and assessment skills, and a positive attitude towards change with the ability to contribute to the evolving nature of our service. The post holder will have regular management and clinical supervision. The post holder provides a high standard of clinical care ensuring safe and effective assessment, treatment and comprehensive discharge planning using highly developed clinical reasoning skills and appropriate assessment tools.

This is a community based service covering the whole of the Lincolnshire County working from 2 Main hubs. This service is to provide a hospital at home service providing initial assessment and continuing health and social care assessment of service users based on their individually assessed needs. Ensuring that an agreed assessment of care needs is implemented which reflects the Trust’s policy and Values. Engage services users and their carers in all aspects of their care and treatment. Working with other professionals such as Occupational Therapy, Pharmacy, Medical professionals and Clinical Psychology in care delivery. Older Adults Home Treatment Team supports service users for around 6 weeks and is a short intensive team. The aim is to reduce the risk, assess and treat the mental health condition and for the service user to be in recovery then refer/sign post to other support services through to safe transfer of service.

Plan, implement, review and improve interventions to meet people’s identified needs and manage their inherent risk. To assess carers’ and families needs and develop, implement and review programmes of support for cares and families. Protect people from abuse, neglect and harm. Enable individuals to develop independent living skills and live well in the community where possible. To work collaboratively and promote effective working relationships with members of the multi-disciplinary team, ensuring effective and appropriate clinical decision-making, within the Older Adult Home Treatment Team and Community Mental Health Teams and with external agencies. To maintain accurate and timely clinical records and to co-ordinate and monitor those of the team. To adhere to  codes of professional conduct and ethics, plus associated legislation. To develop clinical practice having due regard to guidelines of the N.S.F. for Older People, N.S.F for Mental Health, N.I.C.E. and N.I.M.H.E. To demonstrate empowering leadership skills within the Older Adult Home Treatment Team and seek opportunities in local and national area as to promote and develop the profession. To participate in Clinical Governance activities, including: induction, supervision, personal development review, health and safety, risk management and audit. To undertake specific project work or any other duties as negotiated with the Older Adult Home Treatment Team. JD attached.

Qualifications

  • Evidence of specialised continued professional training (degree level) in clinical practice. Registered with a professional body e.g. NMC, UKCP, HCPC, BACP
  • Recognised Professional Qualification in Registered Nursing (RMN,RLDN), Social Worker (CQSW, DipSW), Occupational Therapy RVN or RNLD or RN: 1st Level Registration (NMC) or degree or equivalent.
  • ENB998 / FliPS or equivalent
  • Mentorship Module (degree level)
  • Evidence of specialised continued professional training (degree level) in clinical practice.

  • Management training

Experience

  • Relevant experience of working with adults with mental health needs. Or clear evidence of substantial experience of working with individuals in crisis and skills that are transferable to both community/in-patient.
  • Experience of working with people with severe and enduring mental illnesses
  • Evidence of continuing professional development

Skills

  • Sound knowledge of national agenda for mental health
  • Highly developed clinical reasoning skills
  • Sound knowledge of clinical/risk assessment and understanding of Information Governance principles
  • Highly developed communication skills
  • Delegation whilst maintaining overall responsibility for patients care, where appropriate
  • Demonstrate the ability to lead a clinical team
  • Highly motivated & able to engage with service users & carers to improve outcomes
  • Ability to work independently and collectively
