Main duties of the job

This is a community and hospital-based role providing physical healthcare support and liaison to professionals both within the Learning Disability service and partner agencies including GP practices and services in United Lincolnshire Hospital Trust (ULHT).

The role involves managing a small complex caseload of patients who have a learning disability.  You will work closely with the ULHT safeguarding lead nurse coordinating patient care providing support to the Acute liaison nurses and with primary care colleagues to support Community liaison nurses.

As part of this role there is a requirement to support the validation of GP Quality Outcome Framework Learning Disability Registers annually. You will deputise in the absence of the physical health liaison lead and provide management and clinical supervision to staff within the service and provide support and training in respect of community and acute physical healthcare to the Learning Disability Service.
