Are you a healthcare assistant with experience caring for cancer patients looking for a new challenge? We have a position available for a compassionate HCA who is enthusiastic, caring, works as a team player and is driven to improve the patient experience.
The Cancer Support Worker will assist the Prostate Cancer Nurse Specialists in providing high-quality patient-centered care to those living with prostate cancer.
You will be working closely with other healthcare professionals and liaising with members of the multi-disciplinary team to support the health and well-being of our patients and their families. You will help support patients through their diagnostic and treatment pathways where you will be trained to provide telephone advice, out -patient clinic and ward based support. Supporting patients with their holistic needs assessments to help meet their information needs and signpost them to support services.
You will need to be able to demonstrate good communication, interpersonal, organisational and IT skills.

Main duties of the job
The remit of the Cancer Support Worker will be to support the coordination of care for cancer patients through their cancer pathway under the supervision of a Clinical Nurse Specialist (CNS). This role will be focused around the whole cohort of cancer patients who are referred; it will support patients in navigating their treatment and using the complex health and social care system before and during treatment. Also, it will provide guidance and information to ultimately improve patient experience.
This role will provide a telephone or face-to-face triage service within an agreed scope of practice for people affected by cancer from pre-diagnosis to the end of treatment. There will also be scope to provide outreach support under the direction of the registered practitioner, making every contact count.

Detailed job description and main responsibilities
Under the guidance and supervision of the CNS:
1. To provide a telephone or face-to-face triage service within an agreed scope of practice.
2. Signposting of queries as appropriate to the clinical, information or administrative teams.
3. Ensure prompt documentation of patient/carer contacts in Info flex, on recognising that the key worker has personal accountability for entries to patient records made by others under their supervision so must be informed.
4. Using excellent communication skills, and appropriate tools and procedures, liaising as appropriate with the CNS when non routine and refer complex decisions to the team for assessment and review.
To support the CNS in coordination of care for cancer patients from referral throughout their treatment making every contact count.
To visit patients as in-patients or out-patients as directed by the CNS providing information and support as directed.
Support patients by completing Holistic Needs Assessments and developing Care Plans as pivotal points along their cancer pathway.
To provide a ‘touch point’ for patients to ensure two-way communication during treatment.
To provide guidance, contact and support for cancer patients as they journey through our hospital(s).

- Diploma level qualification or equivalent in relevant field/discipline 5 GCSE grade C or above including maths or numeracy subject or equivalent

Experience
Essential criteria
- Relevant Health or Social Care experience at Band 2 or equivalent Experience of Multidisciplinary Team working Evidence of good communication skills Experience of coaching / teaching patients and carers Understanding of the health and social care environment Experience in the use of data management Experience and working knowledge of computerised hospital patient administration systems
Desirable criteria
