Are you a registered nurse or allied healthcare professional looking for a new challenge? An exciting opportunity has arisen at University Hospitals of Leicester NHS Trust (UHL) to appoint a Macmillan Personalised Care Lead. We have secured 2 years of Macmillan funding to improve patient experience through the ongoing delivery of personalised care and personalised stratified follow up at UHL as indicated in the NHS Long Term Plan (2019).
We are seeking motivated registered nurse or allied healthcare professional with a passion and commitment to improve the cancer pathway and experience of patients living with and beyond a cancer diagnosis.
You will work alongside the Macmillan lead Cancer Nurse to provide strategic and operational leadership across the Trust; the post holder will work within a dynamic team a small team consisting of a personalised care facilitator, personalised stratified follow up manager and administration team to support our clinical in the delivery of personalised care including personalised stratified follow up.
This role will give the successful individual a opportunity to shape, deliver and embed long-term sustained improvements to advance the services we offer to our patients, and to play a key role in the on-going cancer transformation within the trust.

The post holder will be responsible for the delivery of Personalised Cancer Care and Personalised Stratified Follow Up (PSFU) pathways as per the NHS Long Term Plan (2019).
Leading the programme and providing strategic and operational leadership across the Trust. Working alongside Clinical Nurse Specialists (CNSs), Clinical Teams, Allied Health Professionals (AHP’s) and Clinical Management Groups (CMG’s), the post holder will take day to day responsibility for the programme of work to improve the experience of patients living with and beyond cancer at University Hospitals of Leicester NHS Trust (UHL). Fostering and developing strong collaborative working relations internally and externally including stakeholder groups will be key to this role. This will include balancing the challenges of providing high quality, safe services, with excellent outcomes in a way that delivers efficiencies and meets the needs and expectations of patients and the public.
The post holder will represent the Trust and deputise for the Macmillan Lead Cancer Nurse for any matters related to Personalised Care and PSFU and attend internal and external meetings with other service providers as required.
The post holder will provide operational line management to a small group of staff providing credible, effective, and visible professional leadership.

Lead and operationally manage the Programme to ensure service improvement plans are implemented and delivered to time to achieve defined quality outcomes, productivity, access and experience benefits for people affected by cancer, including preparing, co-ordinating and maintaining programme plans.
Maintain appropriate documentation through all phases, providing updates to project plans and reports as required.
To build and establish good working relationships with key internal and external partners across social and health care providers to facilitate high quality patient care.
Engage with Cancer Clinical Teams, managers, administrative teams and other staff to change and improve their services.
Lead on the implementation and surveillance of agreed operational process, service metrics and escalation process. This will be underpinned by written reports, action plans and as required review meetings
Provide and receive highly complex and sensitive information, requiring the post holder to negotiate, influence and act with diplomacy at a high level.
Support the Lead Cancer Nurse and cancer services with interpreting national policy / guidelines and report on best practice to inform and influence service change and improvements in the Trust. Identify and draw on best practice from other organisations inside and outside of health care in designing service improvement programmes.
Proactively manage/ mitigate and advise on risk reduction actions for key risks and issues associated with the programme, ensuring appropriate actions are taken to mitigate or respond to identified risks related to the defined portfolio.
In collaboration with the Lead Cancer Nurse determine clear governance and engagement arrangements including lines of responsibility and accountability.
Co-ordinate and be accountable for the robust implementation and ongoing monitoring of the Personalised Cancer Care agenda, including Holistic Needs Assessment (HNA) and Personalised Care Plans, Treatment Summaries, Health and Well Being Interventions and Personalised Stratified Follow Up pathways.
Support the Macmillan Personalised Care Facilitator in the ongoing implementation and reporting of of the programme including the provision of education & training.
Support the implementation of personalised stratified follow up pathways within the Trust in line with local and national guidance.
Plan and manage the project implementation activity, lead and motivate project teams providing practical consultancy support as needed to facilitate project delivery, monitor and adjust plans as required to ensure that projects meet agreed timescales and budgets and that project objectives are achieved.
Promote the involvement of patients, carers and their families, embracing co-production and co-design across all work streams. Use feedback from people with lived experience to influence the service design and delivery of care. Ensure clinical teams and key stake holders are aware of the outcome of patient / carer experience.
Develop a robust personalised cancer care data dashboard that is produced monthly. Produce briefing documents and papers as requested to present at internal and external meetings where appropriate.
Working with teams lead on the collation of quantitative and qualitative data to provide evidence of productivity (including COSD), outcomes and quality, promoting and supporting audit and research.

- Registered Nurse on the NMC Register or HCPC Registered professional.
- Degree Level Qualification
- Educated to master’s Level in a Health-Related Subject or equivalent demonstrable experience supplemented by specialist training, experience, short courses or equivalent related to Cancer and or personalised care.
- Qualification or equivalent experience in change management, project management methodologies
