An exciting opportunity has arisen for an experienced Nurse to join our clinical team within the Cardiac Rehabilitation service.

Excellent communication and interpersonal skills are essential. Being a registered 1st level nurse with specialist knowledge you will be well-motivated and have a minimum of three years clinical experience at Band 5 preferably within CCU/Cardiology setting.  Having continued your professional development you must also display evidence of management/leadership skills and hold a recognised teaching qualification.  It is desirable to have experience of clinical audit and participation in research.

You will have a clinical caseload within the cardiac rehabilitation team and support the clinical rehabilitation team in delivering new and innovative clinical services by implementing research into clinical practice. The post holder will be working across UHL sites and working in the National Centre for Sports Science Medicine (NCSEM) based at Loughborough University and other non-UHL sites to include Coalville Leisure Centre, New Parks Leisure Centre and Aylestone Leisure centre.

To be responsible for the rehabilitation of patients with cardiac diseases (across all phases of rehabilitation at all UHL sites to include an individual assessment and post rehabilitation patient exits, outpatient classes and education as appropriate).

 

To be a core member of the multi-disciplinary Cardiac Rehabilitation specialist team delivering high quality, clinically effective care and patient experience, utilising research in collaboration with colleagues in everyday practice.

 

Act as the clinical expert and professional resource to the patients and carers and multi-professional team, within their specialist service.

Directly or indirectly influence all aspects of care and management of patients within the service/speciality.

 

Act as a role model and clinical resource demonstrating high standards of care and providing clinical leadership in the specialist area.

 

Support the development and delivery of a programme of practice and professional development for staff within the speciality.

To carry out clinical assessments and final reviews for patients referred to the rehabilitation services and be proactive in the development of rehabilitation strategies. 

 

Working with rehabilitation teams on initiation and planning of new projects to encourage development of the service.

 

Bring about change through the ability to evaluate own practice and that of others.

 

Keeping up to date with recent advances in the field of Cardiac Rehabilitation and to promote the use of evidence based practise.

 

To forge close links and improve communication channels with other members of the health care team to maximise the effectiveness of the rehabilitation

 

Form part of a multidisciplinary clinical team within a wider clinical and research department. You will be accountable to the Head of Pulmonary and Cardiac Rehabilitation. 

 

You will have a role to ensure all aspects of performance, attendance, appraisal and training are addressed in accordance with Trust Policy.

 

You will be responsible for delivering high quality evidence based cardiac rehabilitation in accordance with national and international guidelines and local strategy. 

 

You will assist in developing and delivering rehabilitation in line with published quality standards and emerging/new research. 

 

You will maintain effective communication with the wider multidisciplinary healthcare community, promoting and informing of specialist service and will have highly effective communication with patients, relatives and carers, often with complex needs.
