Job overview

An opportunity has arisen for a Teenage and young adult Cancer Clinical Specialist nurse to join the enthusiastic Teenage Cancer Trust team to be a part of the MDT service to support Teenage and young adults with cancer at the regional cancer centre. The post will be hosted at the Cancer Centre at Lancashire Teaching Hospital NHS Foundation Trust. However this is a network service covering Lancashire and South Cumbria

You will be supported by the Lead Cancer Nurse and and TYA CNS at LTH, TYA Lead Nurse at the Christie the wider national Teenage Cancer Trust Nurses group and the Head of Nursing and Quality (Teenage Cancer Trust). Teenage Cancer Trust will support your professional development and educational opportunities will be provided.

This role will involve supporting patients with complex care needs that require specialist assessment, treatment and care planning. You will need to be an energetic, motivated specialist nurse who possesses excellent communication skills, allowing you to support patients and families through their cancer pathway. In addition to direct patient care the role will involve supporting junior staff & delivering education through ward based teaching

You will be expected to work collaboratively with the network in order to coordinate the best possible care for teenage and young adults with cancer.

 

Main duties of the job

You will be required to work autonomously and exercise a high level of clinical expertise when identifying and caring for the specific needs of TYA patients and their carers. You will need excellent interpersonal skills, senior nursing experience, knowledge of TYA cancers and the ability to work sensitively and collaboratively with diverse teams. By working alongside the TYA team and Site Specific Teams you will promote a cohesive approach to developing TYA care within the region.

The successful candidate will develop excellence in clinical care for Teenagers and Young Adults with cancer and their families being treated within the Designated Hospitals in the Northwest. You will play a key role in developing and improving the quality of patient care in relation to TYA services in line with Quality surveillance measures. You will act as a Key worker to a defined TYA caseload and support existing Site Specific teams to deliver age appropriate care, promoting the philosophy and ethos of Teenage Cancer Trust.

Detailed job description and main responsibilities

  • Provides expert clinical care and support
  • To act as Key Worker for a clinical caseload, working to coordinate care and provide easy access for ongoing support for patients
  • Provide comprehensive  patient core contact for the TYA Service through media available to the patient e.g. face to face, text, telephone, home visit (if necessary), website, e-mails, written information Actively participates in the direct clinical management of teenage and young adults living with cancer
  • Provide comprehensive highly specialist, clinical care, support and advice to teenagers and young adults with cancer (both to the patient and the site specialist teams
  • Effectively communicates with patients and other health professionals
  •  Undertakes research, audit and quality initiatives
  •   Actively involved in service development and personal development
  • Working  with the TYA lead nurse and the  TYA Principal Treatment Centre (PTC) to lead the implementation of TYA services (16-24 years) across Lancashire and South Cumbria  Hospitals, ensuring all patients receive age appropriate high quality clinical care
  • Within the nominated Geographical area , act as a source of advice and clinical expertise, providing a comprehensive specialist role for TYAs with cancer to improve both outcomes and experience
  • Act as a Key Worker to patients who do not have a Clinical Nurse Specialist or Key Worker already in place.
  • Work closely with the TYA Team and the site specific  Multidisciplinary Teams locally and at the PTC to ensure alignment of goals and priorities between national and local modernisation initiatives relevant to TYA Cancer Services
  • Lead change in clinical practice for the benefit of the TYA with cancer to include their carer, multi disciplinary clinical teams in primary, secondary and tertiary care.
  • Ensure patients (16-18 inclusive) are referred to the PTC for treatment at diagnosis
  • Ensure patients (19-24 years) are supported to make an informed decision regarding place of care; either locally within a Designated Hospital or referred to the PTC
  • At all times to promote the philosophy of the Teenage Cancer Trust Charity 

Person specification

Qualifications and Education

Essential criteria
  • Registered General Nurse
  • Educated to degree level
  • Evidence of Masters level learning or prepared to work towards
  • Evidence of continued professional development
  • Training or prepared to work towards
  • Recognised teaching/mentor qualification
  • Experience of TYA or relevant training course.
  • Commitment to ongoing training
Desirable criteria
  • RSCN/RN-Child
  • Recognised Management/Leadership Course
  • MSc in Cancer Care or Nursing
  • Commitment to study TYA Specialist Practice Training
  • Project Management qualifications/ experience

Knowledge and Experience

Essential criteria
  • Knowledge of the needs of Teenager and young adults including those diagnosed with cancer
  • Awareness of primary health care issues affecting a diverse local population of teenagers and young adults
  • Knowledge in the delivery of Health Care Advice and Health Promotion
  • Knowledge of Clinical Governance, quality and risk issues and ability to establish safe systems in practice.
  • Understanding of legal and ethical issues/responsibilities relating to nursing practice, research, and especially with regard to autonomous practice
Desirable criteria
  • Knowledge of safeguarding and vulnerable adult policies and procedures
  • Knowledge of the national TYA groups and national developments in TYA cancer care
  • Experience in haematology/ oncology
