We have an exciting opportunity to join our renal team as a specialist nurse in home dialysis therapies. If you are looking for career development working with renal patients this is an ideal opportunity.

MFT are looking to recruit an enthusiastic individual to support and enhance the development of the home dialysis service. We are interested in hearing from nurses who have at least 3 years post registration renal nursing experience, and a minimum of 18 months working directly with haemodialysis patients. The successful candidate will need to be highly motivated, aspirational, and caring with a clear interest in promoting self-care dialysis. They will be able to work within our well established and growing home dialysis service, supporting both home haemodialysis and peritoneal dialysis patients in the community.

They will be able to work autonomously and make independent decisions whilst contributing fully to the MDT. This is an excellent opportunity to gain further knowledge and experience within this speciality and further develop your clinical skills. There will be opportunities to develop autonomous practice, experiencing high level work and develop specialist skills with support from the renal MDT and specialist nursing team.

Main duties of the job

The post holder will work primarily within the relevant clinical field (Home Dialysis).  The post holder is responsible for delivering immediate care to home dialysis patients and will work primarily within the community in the patient’s homes.  The post holder will prioritise, assess, plan and evaluate care for these patients, will provide direct patient care and have detailed clinical knowledge of the specialist area. The post holder will assist the senior nurses in ensuring that high standards and quality of care are delivered and maintained.

Detailed job description and main responsibilities

Main Organisational Relationships

Patients  on home dialysis and their families.

Patients considering a home dialysis therapy.

Patients training for home dialysis.

Clinical Nurse Leader/Modern Matron, Lead Nurse/Modern Matron, Senior and junior nurses within the department. Education Leads, Consultant and junior medical staff, Support and ancillary staff including nursing students.

Community Nursing Services, GP’s and Social Services.

Clinical Governance team and teams of nursing specialists based in the hospital and outside the hospital setting.

All members of the multi-disciplinary team to ensure a co-ordinated service is provided.

Essential and desirable

Essential criteria
  • Clinically competent in clinical area/speciality including Knowledge of both haemodialysis and peritoneal dialysis.
  • Minimum of 3 years post registration experience in relevant clinical settings, including a minimum of 18 months working directly with haemodialysis patients.
  • Good practical and theoretical knowledge of haemodialysis with well-established renal vascular access needling skills
  • First level registration – RN (Adult).
  • Good analytical and clinical decision-making skills.
  • Capacity to work with colleagues at all levels. Able to relate well with patients, clients, relatives, visitors, colleagues, the multi-disciplinary team and wider Trust staff.
  • Ability to develop a high level of autonomous practice.
  • Competent driver (with 3 or less penalty points).
  • Proven ability to problem solve.
  • Effective organisational skills.
  • Evidence of teaching assessing skills/ability.
  • Time management skills.
Desirable criteria
  • Good practical and theoretical knowledge of Peritoneal dialysis.
  • Evidence of advanced decision-making skills in both haemodialysis and peritoneal dialysis.

You must have appropriate UK professional registration.
