Part-Time - 25 hours per week

An exciting opportunity has become available for a Band 6 Nurse to join the South Lakes Older Adult Community Mental Health Team for 25 hours per week. The team are looking for a dynamic colleague who shares our passion for ensuring that individuals enjoy great care and health regardless of age and diagnosis.

We are looking for a forward thinking, caring individual who has excellent leadership qualities and experience of working with mental health difficulties relating to the ageing process and people who have a dementia diagnosis. Experience in a care home setting is desirable.

One of the main roles of this post is to supervise, educate and support staff, manage referrals and to communicate team needs, and manage complex referrals.

This person must be able to work as a member of a strong multi-disciplinary and a very progressive team.

Main duties of the job

Working within the wider Community Mental Health Team as a senior member of the OA CMHT to provide person centred, evidenced based care to support the needs of service users who are in need of therapeutic intervention and treatment for functional mental health conditions and complexities from a dementia diagnosis. 

To ensure that service users and their carers receive the best quality care and support in line with their changing complex mental health needs.

Working closely with, nurses, OT’s, doctors and the wider MDT to maintain positive health, utilising occupation, formulation, observation and education to provide Dialog+ care plans to support needs and recovery in collaboration with service users and their carers at all times

Qualifications

Essential criteria
  • First Level Registered Nurse RNMH, RMN, Mentor Training.
  • Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
  • Proven track record of post registration clinical experience relevant to the post. Training in Dialog+ Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.

Knowledge

Essential criteria
  • Awareness and understanding of professional accountability, NMC standards and other local and national initiatives.

Experience
