Job overview
The Children’s Community Nurse (CCN) will work collaboratively with other professionals and agencies in the assessment, implementation and evaluation of specialist nursing care to children with long term, life limiting or palliative care conditions.
The CCN will support and contribute to the development of the Integrated Community Paediatric Service (ICPS), helping to ensure that policy and practice is developed and delivered in line with Trust, regional and national best practice.
Provide and promote evidenced based knowledge and skills to manage the health outcomes for CYP and their families and carers across a variety of community settings to safely meet the CYP health care needs.
To act in advisory role to other professionals and staff from other agencies in respect of health care for CYP and families within their area of specialism.
The post holder will support the wider ICPS and participate in the service provision for 24-hour-on-call, which will include at times attending home visits to support end of life care out of hours (supported by other staff).
Main duties of the job

Promote continuous improvement for the CYP and their families by collating their experience, utilising the Friends and Family Test and Patient Experience survey.

Listen to and attempt to resolve problems for the CYP and their families and escalate any concerns to SCCN or Team Leader.

Clinical duties will include; clinic based work, home visiting, administration of medication, teaching and training families and carers, undertaking risk assessments, reviewing acutely unwell children and escalating any concerns identified.

Provide support to SCCN in the management of complex cases and report back any concerns identified.

Provide local support to patients under the care of Oncology services this will include; line flushes, blood taking and administration of low dose cytotoxic drugs.

To actively seek the views and experience of the CYP and use the information to ensure the service is responsive to the CYP and their families.

To support SCCN in End of Life Care in the community if this is the child and families preferred place of care.
