Admission Avoidance Team

We are seeking to appoint a dynamic, confident, innovative and flexible Nurse or AHP with excellent communication and organisational skills. The post is 37.5hours per week and can be offered as either a permanent position or secondment opportunity. 

This post holder will join our existing team of high functioning, highly motivated Advanced Clinical Practitioners, who deliver a wide variety of services including 2 hour Urgent Community Response, Hospital at Home/Virtual Ward, Falls pick up services and dedicated Care Home support, all enable people to be cared for in their usual place of residence. The Advanced Clinical Practitioners operate seven days a week between 08.00 to 20.00.

The post holder will work with partners system wide to provide integrated care across the health economy.  The applicant will have qualified and hold an MSC in Advanced Clinical Practice.

The applicant must be a driver with access to a vehicle for work purposes subject to the provisions of the Equality Act

Essential criteria
  • Registerd nurse , AHP or paramedic
  • Independent non medical prescriber
  • Mentorship course
  • MSc in Advanced Clinical Practice
Desirable criteria
  • NMC practice teacher or equivalent

  • significant experience in a clinical setting
  • • Able to demonstrate advanced theoretical and clinical knowledge and skills.
  • Understanding of clinical Governance and risk management
  • Evidence of carrying out research/audit/ leadership
  • Excellent team working skills
  • Have an understanding of team dynamics and an ability to influence the building of effective working relationships between staff
Desirable criteria
  • • Experience in various acute areas.
  • post registration experience in a relevant clinical setting

Knowledge

Essential criteria
  • • Experience of multi-disciplinary working
  • • Effective communication skills to communicate complex and highly sensitive information.
  • Expert knowledge of dealing with patients in a reactive and unscheduled environment.
  • Knowledge of Virtual Ward models of care delivery include the use of technology to support patients living well in their usual place of residenc

  • Understanding of the current health reforms, commissioning structures and policies

Skills

Essential criteria
  • • Ability to work autonomously, plan, organise, prioritise and delegate appropriate interventions to support staff
  • • Competent in organising own work load.

  • Advanced clinical skills in patient assessment, diagnostics and treatment planning
