Band 5 - Children's Community Nurse - fixed term 10 months (to cover maternity leave)

The Children’s Community Nursing Team is integral to the Ambulatory Model that is offered and delivered within the Tameside and Glossop locality.  The team accepts direct referrals from the emergency department, children’s observation and assessment unit, children’s ward and local GP’s working hard to facilitate early discharge from hospital in addition to providing a rapid response service to prevent unnecessary hospital attendance/admission.   

The Children’s Community Nursing Team  provides nurse-led clinics to ensure that children and young people  with non-acute conditions receive a service that is close to home. 

You must be a registered children’s nurse and have extensive clinical knowledge and skills in caring for children and young people with a variety of needs in acute services to enable you to practice autonomously and use your clinical decision making skills.  

Experience of working within the community setting is desirable. You should have excellent inter-personal and communication skills and the ability to demonstrate sound theoretical evidence based knowledge.

You should be committed to the delivery of high standards of care, and have the ability to work both independently and as part of a multi-disciplinary team. The role includes a large amount of lone working and as such we have robust policies in place to safeguard individual Children’s Community Nurses.  

Main duties of the job

An exciting opportunity has arisen for enthusiastic individual to join our Children’s Community Nursing Team.

You will be working within an established and experienced team and will be involved in the provision of and support the delivery of skilled paediatric care to children and young people with acute/long term conditions and palliative/end of life care.
