An exciting opportunity has arisen for an experienced nurse in the areas of tissue viability/ leg ulcer and lymphodema. 

The post holder will work to support the current Service Lead and Band 7 Team Leader to clinically and operationally manage the Wound Care & Lymphoedema Service across the acute and community setting of Tameside & Glossop. The role requires the ability to facilitate and embrace change and be committed to team working. The post holder will also act as a specialist support for the tissue viability element of the service

Main duties of the job

The post holder will take the leading role in the team management of a specialist nurse team to support case load oversight, assessment, planning and implementation of programs of care with all areas of Tissue viability service including lower limb, Lymphoedema and Leg ulcer, Surgical site infection pressure ulcer management and prevention.

The post holder will work independently within the Tissue Viability/Lymphoedema team/leg ulcer service working within community and hospital settings, providing specialist wound care advice, organising workload, team management, delegation of tasks, promoting clinical excellence with a specific focus on the assessment and management of all aspects of the Tissue viability service. Exercising leadership and judgement to ensure good clinical practice and high standards of care are maintained in order to meet service demand.

Detailed job description and main responsibilities

SECTION 1- MAIN RESPONSIBILITIES:


The post holder has continuing daily Tissue Viability team responsibility and accountability without direct supervision from the Tissue Viability lead nurse. 
Overall case load accountability oversight and in depth knowledge of the complex patients on the TV caseload. 
The post holder will act as a point of contact to guide and mentor junior members of the Tissue viability team, contributing to clinical practice both directly and indirectly. 
The post holder will take the lead in Quality improvement projects, including teaching programmes.

The post holder will be expert knowledge in Tissue Viability to assess, plan, implement and review routine through to complex issues using research and evidence based knowledge to underpin practice in the field of tissue viability, lower limb issues including lymphoedema, including compression therapy strategies and multi layer bandaging where appropriate. 
To supervise, lead, inform and delegate to staff as appropriate to the needs of theservice by utilising nursing skills and resources effectively as directed by the lead nurse. This will include: chronic odema management, complex wound assessment, doppler assessment, leg ulcer management and pressure ulcer prevention and management.

Apply Specialist, analytical and judgemental skills to assess and interpret acute,chronic and palliative related skin conditions, taking appropriate actions to prioritise patient’s needs.

Demonstrates a high level of intuitive and creative problem solving skills to facilitate patient centred care: rationalising information to create patient concordance by using a high level of communication/negotiation skills. 

Oversees the performance and development appraisal process within the team and undertakes review of junior staff under supervision of the lead nurse.

Prescribe efficiently and effectively from the Greater Manchester wound care Formulary.

Takes the lead in the investigation of complaints and safeguarding concerns in accordance with the level of experience and encourage comments and suggestions on improving the quality of the service.

Takes the lead to ensure that accidents, complaints and unusual incidents are reported in accordance with the policy.

Takes the lead in coronial inquests.

Analyses caseloads and constantly reviews work patterns as required.

Takes the lead in systems of clinical governance to achieve quality improvements i.e.clinical audit and evidence based practice.

To support and develop staff using CPD and to identify future leaders.

Essential criteria
  • Registered General Nurse
  • Post registration experience in wound care
  • Relevant post registration Leg ulcer management course or equivalent
Desirable criteria
  • Nurse prescriber V100, V150, V300
  • Specialist practitioner district nursing qualification
  • Conservative sharp debridement

Experience

Essential criteria
  • Experience of working at a senior level in the management of patients with complex tissue viability/lymphedema needs
  • Wound management skills: Topical negative pressure application, Doppler and compression bandaging
  • Experience of managing staff and resources in a variety of settings i.e. Acute and Community
Desirable criteria
  • Knowledge of lymphoedema assessments and techniques
