Bed Management Team Leader

We are looking for a Team Leader to join our Bed Management Team based in Sevenoaks.  

We are a team of clinicians with extensive knowledge and experience within clinical care. 

We receive and triage referrals for patients referred to the trust community hospitals for rehabilitation using the GIRFT method.

We provide robust daily staffing support to community hospitals in the trust network, this allows us to ensure that the ward acuities and staffing numbers are at a safe level for the planned admissions and providing ongoing care to our patients.

In your role as Team Leader you will provide effective and compassionate leadership to the bed management team and be responsible for the timely, appropriate and safe admissions of patients to the community hospitals.

You will liaise with acute hospitals, community hospitals, social and health service colleagues in order to promote excellent multi-disciplinary working to achieve effective flow of patients between care settings to enable the most appropriate patient centred care for patients

  • Registered health care professional (NMC or HCPC registration) / minimum of 3 years of experience in health improvement environment
