Main duties of the job
To assist the Community Parkinson’s Disease Specialist Nurse in the provision of the specialist nursing
- To manage a caseload of clients requiring specialist Parkinson’s Disease nursing, within the community.
- Undertake assessment and treatment of clients within both clinic and community setting, who have
complex / and or chronic presentation requiring specialist skills; and determine profession specific andmultidisciplinary rehabilitation interventions. - To provide advice, education and support to patients, their carers and healthcare professionals, and assist in
coordinating treatment and care. - To work across professional, departmental and Trust boundaries to support the delivery of a patient-centred
seamless service. - To work in close collaboration with the Community Neuro & Specialist Services, delivering and evaluating
care pathways for clients with long term neurological conditions.
This is a full-time role and a car driver and access to own vehicle is essential to the role.

Qualifications
Essential criteria
- (NMC registration ) minimum of 3 years of experience in health improvement environment).Health related degree and evidence of relevant post graduate development at Level 6
Desirable criteria
- PRINCE 2 or equiv project management training PTLLS/DTLLS, PGCE or Certificate in Education
Experience
Essential criteria
- Minimum of 3 years in area of practice
Desirable criteria
- Experience of working with Parkinson's patients, Experience of working in the community, experience of leading in teams, experience of IT systems & packages
Special knowledge & Expertise
Essential criteria
- Understanding of National Service Frameworks and NICE guidance to relevant to care setting and its application across health care services Evidence of continued relevant professional development and implementation in practice within the last 12 months
Skills & Abilities
Essential criteria
- Understanding of training needs in relation to post Ability to manage time effectively, priorities and work under pressure to meet deadlines Ability to work effectively with a variety of people at all organisational levels across different sectors including areas of social deprivation Ability to communicate complex, sensitive information to individuals and groups Strong influencing and negotiating skills
