Health Visitor- Greenwich - Band 6

£40,701 - £48,054 Inc HCA per annum pro rata 

37.5 hours per week (Full time and part time available)

Bromley Healthcare are seeking qualified and student Health Visitors who are keen to develop their leadership skills and sphere of influence and are keen to shape the future of Public Health Nursing in Bromley, Bexley and Greenwich.

We are invested in the development of Health Visiting and School Nursing and this is underpinned by our Public Health Nursing strategy and training programme for all roles within the PHN teams.

All Health Visitors and Community Nursery Nurses have corporate membership with the Institute of Health Visiting (iHV) which offers a wealth of professional training programmes and resources to support excellent practice. The iHV has been commissioned to deliver the Leading Excellence in Practice programme to all Health Visitors and School Nurses over the next 18 months.

Our vision is to develop a progressive, inclusive and sustainable workforce that is able to meet the diverse and changing needs of the families whom we serve, and we have created developmental roles who are completing Public Health modules with Canterbury University and will go on to train as Specialist Community Public Health Nurses.

As a health visitor working in Greenwich you will provide quality face to face contacts for families for all key contacts, work in a diverse borough following the Healthy Child Programme.   Health Visitors have regular management, safeguarding, clinical and restorative supervision and support throughout your career and a chance to develop and grow with Bromley Healthcare.  

Work as part of an integrated team and lead on the delivery of the healthy child programme

· Identify health needs through an ecological assessment and in partnership with families, agree personalised plans to address the needs.

· Delegate aspects of care to the Community Nursery Nurses and Staff Nurses within the skill mix team.

· Engage and participate in service improvement.

· Continuously learn to develop your own skills and those of others.

· In partnership with clients and other partners, assess healthcare needs to develop and evaluate programmes to address the health needs of the population

· Work in collaboration with other health care and service providers to ensure seamless delivery of services for clients and communities

Compensation is contingent upon NHS experience and current banding/pay point.

To deliver the Healthy Child Programme by:

  • Carrying out ongoing health needs assessments for all children and families within the community, using professional judgement to allocate each family to the correct level of health visiting care, Universal, Additional Support Offer, Intensive support and Intensive Support Offer for Young Parents pathways.
  • Overseeing the delivery of Universal service key touchpoints by skill mix team members ensuring they are within required timescales, according to the service model, to meet KPI targets.
  • Delivering Additional Support Offer, Intensive support and Intensive Support Offer for Young Parents packages of care to families.
  • Using Outcome measures to evaluate the effectiveness of care delivered, planning and implementing revised packages of care, where required, and closing or escalating packages of care as required for each child.
  • Participating and where appropriate, leading on the Early Help process for children, ensuring timely assessment and that coordinated care is delivered to address the needs of the child and family.
