The post holder will be responsible for the assessment, planning and provision of specialist nursing, advice and carries out specialist nursing procedures. The post holder will provide clinical supervision to other staff, students and may undertake research, lead clinical audits in own specialist area. Ensuring the safety and wellbeing of service users and always working in a manner that promotes dignity and improved patient experience through the adoption of person centred care principles.

Main duties of the job

Demonstrate appropriate practice and specialist advice in the assessment, planning and implementation and evaluation of patient care. This includes history taking, physical examination, and the requesting of clinical care management changes.

Act as an advocate for the patient within the department, to ensure a patient orientated approach to the delivery of care and to meet agreed standards

Recognise and avoid situations that may be detrimental to the health and wellbeing of the individual.

Is wholly accountable for his/her practice in line with the Nursing and Midwifery Council’s Code of Professional Conduct and takes every reasonable opportunity to sustain and improve his/her knowledge and professional competence.

Attend and participate in multi-professional team meetings/steering groups.

Organise own work and manage own caseload and practice, identifying and maintaining supportive networks for self and other staff members.

Detailed job description and main responsibilities

Independently manage a group of patients within the speciality whilst working as part of the multi-disciplinary team, delivering specialist individualised and personalised direct patient care.

Demonstrate appropriate practice and specialist advice in the assessment, planning and implementation and evaluation of patient care. This includes history taking, physical examination, and the requesting of clinical care management changes.

Be responsible for assessing and recognising emergencies in the specialty.

Act as an advocate for the patient within the department, to ensure a patient orientated approach to the delivery of care and to meet agreed standards

Recognise and avoid situations that may be detrimental to the health and wellbeing of the individual.

Is wholly accountable for his/her practice in line with the Nursing and Midwifery Council’s Code of Professional Conduct and takes every reasonable opportunity to sustain and improve his/her knowledge and professional competence.

To be responsible, for the maintaining a safe environment for patients, visitors and other staff within the clinical setting.

Ensure that sensitive patient records are stored appropriately

Support and enable patient and those important to the person to make informed decisions relating to their treatment and management.

A key member of the multi-disciplinary team in supporting patients and those important to the person during the delivery of bad news. (Pre and post-delivery of the news)

To provide leadership, support and guidance to staff in monitoring the planning delivery and evaluation of individualised specialist care.

To be an effective communicator with patients/those important to the person/families regarding treatment plans as well as symptom management.

To promote high quality clinical care and assist in the development of the specialist service, through use of evidence based practice and clinical standards.

Provide a seamless, high quality service from referral through to assessment, treatment and review.

Ensure that patients receive high quality clinical care and a good patient experience, having regard for their customs, religious beliefs and doctrines.

The promotion of best practice in the care of people at their end of life with the aim of enhancing the patient’s and those important to the person’s experience through evidence based practice.

 

Assist the Tissue Viability Specialist Lead to maintain and improve current pathways.      

 

Participate in Audit as required. Audits require evaluating the impact of Local and National strategies, staff training and awareness.

To assist others with the continued evaluation of the service ensuring key National and organisational guidance are incorporated and embedded.

 

Interpret information and take appropriate action whilst leading others to do the same.

Lead on and participate in key developments relating to specialty so that high quality patient focused services are promoted.

Attend and participate in multi-professional team meetings/steering groups.

Organise own work and manage own caseload and practice, identifying and maintaining supportive networks for self and other staff members.

To be involved in the development, assessment and implementation of guidelines, protocols, policies and standard operating procedures

To deputise for the Clinical Nurse Specialist Lead in their absence

Be an effective communicator and be a professional role model in liaising with the community and other members of the healthcare team.

Person specification

Qualifications

Essential criteria
  • RGN/RN (Adult) Level 1, BSc / 1st degree (health related) or equivalent qualification / training / experience
  • Mentorship / teaching qualification
  • Evidence of continuing professional development

Knowledge & Skills

Essential criteria
  • Minimum of 2 years’ experience post registration
  • Demonstrates specialist expertise underpinning theory acquired by continuous professional development.
  • Up to date knowledge of current clinical professional issues
  • Ability to assess, plan, implement and evaluate specialist care delivered to patients/clients and give relevant information /advice to patients and those important to the person.
  • Good communication skills and ability to deal with sensitive information. To include dealing with distressed/angry relatives and challenging situations.
  • Demonstrate an awareness of the clinical governance and risk management and how this affects their role and service.
  • To be able to decide how best to achieve expected results, whilst acting within clearly defined policies/procedures/and codes of conduct.

Experience

Essential criteria
  • Significant post registration experience.
  • Experience within wound care
  • Experienced and competent in the management of patients within the specialty.
  • Experience of teaching/ assessing /supervising others in a clinical setting.
  • Experience in co-ordinating the shift and task allocation.
