We are recruiting for a Cancer Support worker who shares our ambition and vision to deliver the Best of Care by the Best People and has the drive to take us forward, by ensuring that the Trust continues to be the first choice for patients and our staff.
Main duties of the job
The Cancer Support Worker’s role (CSW) supports the coordination of care for cancer patients throughout their treatment pathway. The CSW works with the Advanced Nurse Practitioner and Clinical Nurse Specialists and the multidisciplinary team (MDT) to provide support, advice and assessment for Acute Oncology (AO), Cancer of Unknown Primary (CUP) patients and Malignancy of Unknown Origin (MUO). The CSW also helps support follow-up pathways and works with the Macmillan Personalised Care and Support Facilitator to implement and develop Health and Wellbeing Information and Support.
The CSW works under the guidance and supervision of a registered practitioner, assisting the AOS team and the wider MDT by undertaking clinical and administrative tasks in accordance with relevant procedures, guidelines and policies.

Detailed job description and main responsibilities
The CSW will undertake specifically identified tasks for which they have been trained, can demonstrate skill and understanding and are assessed as competent.
These include, but are not limited to;
Communicating with cancer patients, their family, friends and carers, both face-to-face and on the phone
Liaising effectively with the MDT and other healthcare professionals to provide high-quality coordinated patient-centred care
Providing evidence-based patient information according to patient need
Assessing patients’ emotional and practical needs and responding appropriately
Undertaking Holistic Needs Assessments and making appropriate referrals
Providing support for administration and management of Health and Wellbeing Information and Support
Accessing appropriate IT systems
Signposting patients to resources and support for self-management
Assisting in coordination of patient treatment pathways
Assisting in coordination of care for patients on a follow-up pathway
Maintaining database to monitor workload
Recording of nursing activity
Auditing and evaluation of follow-up pathways, Health and Wellbeing Information and Support and patient outcomes

Qualifications
Essential criteria
- Educated to GCSE A level or equivalent AND NVQ Level 4 or equivalent in Healthcare
- OR equivalent experience across a range of health or social care settings at AfC Band 3 or above
- Care Certificate
Knowledge
Essential criteria
- Knowledge and previous experience of using hospital computerised systems
- Insight into cancer and related issues
- Understanding of health and social care environment
