Job overview

An exciting opportunity has become available for a Band 7 Senior Practitioner to take on the clinical leadership responsibility for the Crisis Home Treatment and Rapid Response Team 24/7 to a team of clinicians from different professions.

If successful, you will ensure a high-quality service is provided and care is delivered to all service users on the HTT and RR caseload who are experiencing a Mental Health Crisis, by the Trust’s policies and procedures and the care programme approach. 

The post holder must hold a full, valid UK driving licence and have access to a car/vehicle to use for business purposes (unless you have a disability as defined by the Equality Act 2010)

Main duties of the job

The post holder will contribute to the implementation of clinical and interdisciplinary strategies to ensure the delivery of a high standard of care and service objectives and to the overall corporate objectives of the organisation.

The post holder will work as a member of the Crisis Home Treatment Team ensuring team clinical performance is optimised and meeting agreed KPIs and ensure that CRHT is a responsive service providing timely assessment and offering a range of home treatment interventions as an alternative to hospital admission. 

The post holder will have robust working relationships with colleagues within the acute and community settings; liaison psychiatry, place of safety, inpatient teams so seamless acute care is provided for service users.

The post holder will work in collaboration with colleagues from the community recovery care group, the local authority, primary care and other key stakeholders to ensure effective care delivery and timely onward care at point of discharge.

  • Nursing registration or HCP equivalent
Desirable criteria
  • Mentor Qualification or HCP equivalent

Experience

  • Considerable experience of working within acute/community adult services with experience of working at a band 6 for min 2 years

  • Considerable experience of working in a CRHT/Liaison Team
