Community Healthcare Assistant - Twilight Service - Band 3

Across Bromley Borough

Bromley Healthcare has an exciting Community Healthcare Assistant (HCA) opportunity working within the Twilight Nursing Team. The HCA is a valued and integral member of the Nursing Team and is responsible for ensuring the effective delivery of nursing care as directed by the responsible registered nurse and in accordance with Bromley Healthcare policies and procedures.

Candidates should be flexible, motivated and able to work within a changing environment.

Proposed interview date week commencing 4th November 2024 

Main duties of the job

The successful candidate will undertake delegated nursing tasks in line with their clinical competence. Applicants should have experience/competence to complete the following:

Administration of eye drops

Blood glucose monitoring

Taking and recording blood pressure

Non-complex wound care

Catheter care

Insulin administration

Recording Observations

Recognition of a deteriorating patient

Set up and take down of enteral feeds

Please note that you will need to provide documentation proof of a care certificate and NVQ level 3 certificate.

Please note that you should have a current Drivers License and access to a car for work. Shifts are between 18:30 - 22:30.

Detailed job description and main responsibilities

Professional
 To ensure that all duties undertaken are delegated from a registered nurse.

 To work within own competencies at all times

 To undertake required training.

 To remain up to date with mandatory and statutory training requirements.

 To take responsibility for own professional and organisational knowledge and to practice according to Bromley Healthcare Policies and Procedures.
Clinical

 Participate in the appropriate care of patients as indicated by the responsible nurse and as indicated in the care plan, reporting back daily or as required.

 Recognise the patient’s right to privacy, dignity and confidentiality, showing due regard to patient’s property.

 Participate in palliative care and care of the dying.

 Participate in the care of patients with long term conditions.

 If competent, undertake reviews of patients with long term conditions e.g. diabetes, following BHC guidelines and protocols, and reporting back to the senior nurse.

 Engage in health promotion activity, giving life style advice to patients and carers according to Bromley Healthcare policies, procedures and guidelines.

 Following training and assessment use prescribed equipment for moving and handling patients.

 Maintain accurate and up-to-date records on all patients using electronic and paper records.

 Participate in team, service and organisation meetings and contribute to service developments as requested

 Effectively communicate as required with patients, carers, the public, staff, GP practices and other key stakeholders by a number of methods including telephone, email, fax etc.

 In accordance with delegated tasks, organise and manage own workload.

 Submit returns on a regular basis.
Administration

 To effectively use electronic databases and information systems.

 To ensure the safe-keeping and confidentiality of patient information, both verbal and written.

 To comply with organisational and service policies and procedures

 To report all complaints, incidences and accidents immediately to the line manager and document such occurrences in accordance with organisational policy.

 To work alongside colleagues to ensure cover is available for planned absences and to provide cover for colleagues during holiday periods and staff sickness

Person specification

Qualifications

Essential criteria
  • NVQ3 or equivalent.
  • Care Certificate
Desirable criteria
  • Additional relevant healthcare courses eg: basic food hygiene, first aid.

Experience

Essential criteria
  • Experience of working in a health care setting
Desirable criteria
  • Experience of working with people with long term conditions or older people.
  • Experience of working in the community

Specific Skills

Essential criteria
  • Practical nursing skills e.g. phlebotomy, catheter care, basic observations including temperature, pulse, respiration rate and blood pressure.
  • Ability to record care accurately and contemporaneously.
  • Ability to work without direct supervision and within time constraints
  • Valid UK Driving Licence and the use of a vehicle for work purposes and the ability to competently use a ‘satnav’.
  • Shows empathy and sensitivity to the needs of patients carers and families
  • Good communication skills – verbal and written

  • IT literacy including use of ipads.
