Job overview

The Home First team is a collaborative health and social care initiative designed to offer supported discharge from hospital or to provide an integrated approach to prevent hospitalisation. The service started in Thanet in October 2023 and will expand to cover East Kent in 2024. Early evaluation shows that the service has a positive impact on people’s outcomes, with 51% of people independent on discharge.


The service operates 7 days a week, 8am to 8pm.

This role requires the post holder to travel to sites across a wide geographical area.  In your application please outline how you intend to meet these travel requirements

Main duties of the job

The Home First team is a collaborative health and social care initiative designed to offer supported discharge from hospital or to provide an integrated approach to prevent hospitalisation. The service started in Thanet in October 2023 and will expand to cover East Kent in 2024.

You will be responsible for growing and managing the service, continuing the service evaluation and working with stakeholders across the health, social and voluntary sector to explore and agree the best patient pathways based on right staff, right skills, right place and time principles.

Person specification

Qualifications

Essential criteria
  • Degree in Nursing or Nursing related subject or for AHP first contact practitioner qualification at Degree level.
  • Current state registration
  • Post registration qualification at degree/ masters level.
  • Management experience Leadership qualification or equivalent.
Desirable criteria
  • Experience / qualification Long-Term Conditions, Transformation and Quality Improvement experience.
  • MSc Management qualification

Experience

Essential criteria
  • 5 years minimum post registration experience
  • Experience of managing and reporting Adult Protection alerts
  • Evidence of enhanced clinical skills.
  • Experience in medicines management.
  • Demonstrable experience of working across traditional boundaries.
  • Experience in LTS/Complex care management.
  • Experience of managing and setting clinical treatment plans for patients with complex multi-pathology.
  • 3 years minimum at senior clinical level within relevant setting.

Knowledge

Essential criteria
  • Preparation of Mentors or equivalent teaching qualification.
  • Excellent communication skills – verbal and written.
  • Excellent inter-personal skills.
  • Experience in establishing and leading nurse therapy/social care led services.
  • Ability to blur boundaries and move between acute and community settings.
  • Full understanding of cultural differences.
  • Very good teaching skills;
  • Proven ability to function well as part of a team.
  • Ability to interpret very complex written and statistical information.
  • Ability to develop policy and practice frameworks as requested.
  • A clear understanding of clinical governance issues and controls assurance standards.
  • Wide knowledge and understanding of current health policy.
  • Clear understanding of the modernisation agenda across health and social care to influence service development and achievement of targets.
  • Ability to interpret impacts of national policy and act accordingly.

Skills

  • Excellent communication, negotiation and influencing skills.
  • Skilled in time management, prioritising workload and self management.
  • Strong evidence of effective leadership and management skills
  • Clear Police check.
  • Very good IT Skills.
  • Ability to travel to all sites within the Trust area.
