Job overview

We are looking to add to our trust wide specialist nursing team within stoma care.

We are small specialist team in East Kent but quite a large stoma care nursing team by other hospital standards.

We are a team of nurse specilaists, support nurses and administrative staff working trust wide including a community provision of support for people living with a stoma.

We can offer you a rewarding and supportive role within specialism of stoma care to develop your skills and  to offer a specialist source of support and expert knowledge to our ostomate community.

Your role would be central to providing a high quality pathway of co ordinated care to patients requiring stoma formation due to colorectal or urological disease.

Supporting and working with patients to achieve independent, successful living is the ultimate, key aim of this role alongside supporting those with more challenging chronic situations using evidence based treatment pathways and the wider MDT to support individuals.

The successful candidate will be required to work trust wide at times  with a focus in the community and GP clinics as well as hospital based clinics and in patient care.

We align ourselves with the trust values at all times and aim to deliver a seamless pathway of care from the pre operative phase to life long independent living with a stoma.

 

Main duties of the job

Working with the wider acute and community MDT's and the patient groups in alignment with the Patient Agenda ("My Stoma") Dr. Derek Mitchell, to provide a high quality pathway of standard and bespoke care to individuals living with a stoma, and the challenges that this may bring.

In patient ward and department support, alongside hospital clinic provision. Being involved with the acute MDT meetings to agree patient care and highlight any risks or challenges to stoma formation surgery, to increase and/or protect quality of life for individuals.

Providing a community support pathway for post operative or housebound patients.

Working closely with GP practices and medicines management team in the local ICB to provide community clinics for annual review of all stoma patients.

To be an advocate for individuals where required and to also support  carers and relatives of those living with a stoma.

To empower and promote independent successful living with a stoma at all times. 

To provide specialist treatment and advice (evidence based) along with management plans regarding stoma related conditions, underpinned by theoretical knowledge and and relevant practical experience.

Experience of acute, general and colorectal surgical nursing.
Some experience of Stoma care 

Ability to learn to work as an autonomous specialist nurse, demonstrating advanced clinical practice, prioritisation skills and evidence based care.

Qualifications

Essential criteria
  • Degree in nursing,
  • Stoma Qualification/experience
  • Acute surgical/colorectal experience
Desirable criteria
  • Stoma Experience or NVQ/foundation course
  • 6-12 months at B6 position within surgery/stoma care

Skills

Essential criteria
  • minimum 4 years/significant experience of working in acute surgical/colorectal speciality
  • Clinical credibility of evidence based practice
  • Excellent interpersonal skills
  • Positive patient experience and outcome focus with evidence
  • Considerable ability to work within a diverse MDT
  • Significant ability to plan and manage enhanced recovery of patients
Desirable criteria
  • Expierience of providing nurse led clinics
  • Community expierience
  • Stoma link nurse experience
  • Pre and post op management of new stoma patients

Knowledge

Essential criteria
  • Knowldege of colorectal and urological pathways, locally and nationally
  • Considerable knowledge of privacy, dignity and psychological support required for stoma patients
  • Knowledge of post operative stoma complications
Desirable criteria
  • Considerable expertise in the management of post operative and chronic stoma complications
