Job overview

We are currently looking for a Band 6 Registered Nurse to work on a Bank agreement for our Physical Health Team based in Braintree, Essex

The post holder will work in close collaboration with the wider Physical Health Team to support the delivery of physical health assessments to people with severe mental illness. The Physical Health Team SMI provide physical health and wellbeing assessments within GP surgeries, with the aim of promoting healthy lifestyles, and to assist people to access the support they need to recover from their mental health and physical health problems. They will also support the locality in ensuring the Severe Mental Illness (SMI) registers are updated.

Main duties of the job

Provide high quality clinical support to junior colleagues and to act as a clinical expert. This means working in accordance with evidenced based care and interventions in conjunction with locally agreed locally agreed protocols and policies. 

The post holder will contribute to assessment of both physical and mental health needs. With a focus on improving the physical health of people of severe mental illness, the post holder will understand the relationship between physical and mental wellbeing.

Have a good understanding of the normal parameters of physical and mental health, to be responsive and proactive to deteriorating health.

Provide effective professional support to the physical health team, including sharing best practice and role modeling to all staff.

Ensure appropriate physical health observations and blood tests are taken and actioned for all patients who are eligible for an annual physical health check. This will involve liaison with GPs, practice nurses, practice managers and other primary care staff.

Assess patients holistically, and to support junior members of staff with the assessment of lifestyles, exercise, activities, mental health, nutrition, smoking, alcohol, drug use, and sexual health. Giving advice on healthy lifestyles following evidenced based practice and signposting our service users to other services. 

Facilitate referral to external services as required including social services and secondary mental health services.

Detailed job description and main responsibilities

Clinical Responsibilities

·       To oversee the systematic review of the SMI registers, to ensure eligible patients are identified for a comprehensive Physical Health check and invited to an appointment to the physical health clinic.

·       To support the smooth running of the physical health clinics, including supporting junior staff with booking appointments, physical health/wellbeing assessments, and the creation of care plans and follow ups as required.

·       To be aware of the sensitive needs of people with a severe mental illness, and to understand the impact of stigma, and unconscious bias in own practice and the practice of others.

·       Will be fully conversant and ensure compliance with NMC Code and Standards of Practice, as well as relevant legislation, policies and procedures.

·       The post holder is required to maintain current registration with the NMC.

·       Provide practice supervision and support for nursing students and student nursing associates on placement in the service.

·       To be aware of the sensitive needs of people with a severe mental illness, and to understand the impact of stigma, and unconscious bias in own practice and the practice of others.

Working relationships 

·       To work under the direction of the Nurse Team Leader/Service Manager

·       To work and liaise directly with primary care professionals, Primary Care Networks, and wider health and social care services as required.

·       Form effective working positive relationships and clear communications with other professionals within the service areas, service users, carers and across any other internal and external stakeholders.

Service requirements        

·       Be able to pre-empt changes in demand on the service and act accordingly to prevent disruption to service delivery and to ensure responsive service provision.

·       To support audit and data collection for service and take a role supporting service improvement.

·       Promote the inclusion of people with severe mental illness into main stream services. At all times, to actively ensure that people with learning disabilities are included in and have access to service development and improvement planning.

·       Will be aware of all policy documents and legal requirements and General practice local procedures pertinent to the post and ensure that these are adhered to.

·       Driving required unless you have a disability as defined by the Disability Discrimination Act 1995

·       Travel to all existing and potential sites within HPFT, Mid Essex primary care and general practice sites.

·       The post holder is based within a HPFT site and will be expected to visit people at a variety of public places or voluntary organisations, GP practices and in people’s homes.

Person specification

KNOWLEDGE, TRAINING AND EXPERIENCE

Essential criteria
  • Registered Nurse – Mental Health/Learning disability/General. Current registration with NMC
  • ENB 998 / Preceptorship and or Practice Supervisor and Assessor Training.
  • At least 3 years’ experience post registration at band 5 & Community nursing experience

AREAS OF EXPERIENCE AND KNOWLEDGE

Essential criteria
  • Experience and skills in management of stressful situations ensuring recognition and support to staff and service users
  • Detailed awareness of relevant evidence base relating to physical health in SMI population – The Lester Tool, Doh Improving the physical health of people with mental health problems: Actions for mental health nurses.
Desirable criteria
  • Experience of working in clinic type settings, e.g Clozapine, Depot/LAI or physical health.

PHYSICAL SKILLS

Essential criteria
  • Car driver (unless you have a disability as defined by the Equality Act 2010 which prevents you from driving
  • Ability to travel and work flexibly across different sites in Essex and occasionally Hertfordshire.
