An exciting opportunity has arisen within in Essex to be part of a new Physical Health Team providing services to people with Severe Mental Illness. HPFT are looking for a Registered Nurse with a passion for improving the physical health and wellbeing of people with mental health difficulties, within the local community.

This post will join a range of new roles within in primary care and will be a core feature of delivering excellent care to people in the local community. This post is an ideal opportunity for those looking to take the next steps in their career and applications from nurses working at band 5 whom are looking for career progression are encouraged.

The successful candidate will work alongside primary care under the clinical supervision and oversight of the team leader, service manager, senior service line lead and other nursing professionals.

The primary responsibility for this post is to provide a comprehensive and systematic annual health check to those on the Severe and Enduring Mental illness register (SMI) and provide nursing support and clinical expertise for the physical health team, developing working partnerships with primary care and ensuring best practice within mental health and physical health promotion.
