We are excited to announce a new opportunity within our Hampshire and Isle of Wight All Age Continuing Care Mental Health and ABI Team. We are seeking self-motivated professionals who share our values of being person centred, honest, courageous, innovative, and collaborative and who are passionate about improving life opportunities for people who have existing or newly diagnosed mental health condition and who require S117 aftercare to allow them to live full and meaningful lives in the community.
Main duties of the job
Participating in complex and detailed information analysis of specific projects/reports requiring high levels of concentration. This includes the ongoing review of enhanced care packages – evidence for ongoing requirements fro clients entitled to S117 aftercare.
Conducting standard annual reviews and right-size longstanding packages of care -
Supporting the MDT to screen and approve new funding applications.
Attending discharge planning meetings, CPA’s, CTR’s
Supporting and managing complex duty cases.
Provide some proactive case management to individual complex and rehabilitation clients.
Facilitating timely discharges from acute hospitals and placements where clients are no longer gaining benefit from that service.
Supporting with safeguarding
Supporting with setting up of PHB's.

Detailed job description and main responsibilities
To work within the NHS All Age Continuing Care team with responsibility for co- ordination and delivery of needs/eligibility assessment, case management and review of patients under the relevant frameworks for NHS Continuing Healthcare, Funded Nursing Care, Fast Track, Children’s Complex Care and All Age Continuing Care.
The post holder will use specialist knowledge and clinical skills to ensure that cases and outcomes are managed in a consistent, equitable, safe and cost effective, ensuring appropriate and proportionate application and interpretation of the relevant frameworks and guidance
The post holder will require good verbal and written communication skills to communicate with patients, their families and other professionals.
The production of accurate, detailed and comprehensive reports is essential.
The post holder will work closely with hospitals, community services and care providers to develop systems and processes to facilitate a smooth assessment and referral process.
The post holder will review the clinical needs of the patient with regard to appropriateness of the placement, level of care being provided, safeguarding and entitlement to NHS All Age Continuing Care.
Be a key member of the team as well as supporting effective communication and stakeholder management, both internally and externally.

Knowledge, Training and Experience
Essential criteria
- Registered Health or Social Care Professional with current registration with professional body i.e. NMC, HCPC or SWE. including AMHP’s and mental health social workers or significant relevant experience (LD/MH/LDAP teams only).
- Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in health or social care.
- Experience of assessment of health and social care needs – care planning and coordination.
- Knowledge and relevant experience working with individuals who are autistic and/or have learning disabilities and/or mental health issues (LD/MH/LDAP team roles).
- Knowledge of complex neurological conditions and experience of working with clients with complex brain injuries. Specific knowledge of the National Service Frameworks and appropriate national policy related to rehabilitation of people with acquired brain injury/ neurological dysfunction (Specialist Rehabilitation / ABI team).
- Ability to work effectively under stressful conditions in a rapidly changing environment.
- Demonstrable experience of care/case management.
- Knowledge of and experience of NHS Funded Continuing Health Care criteria – guidance and policies and other legal frameworks as required (Children and Complex Care, s117 Aftercare, Mental Health Act).
- Good knowledge of Mental Capacity Act and relevant legislation.
- Experience in communications and stakeholder management.
- Effective working knowledge of Microsoft software.
- A good understanding of the health and social care environment and roles and responsibilities within it.
Communication and Relationship Skills
Essential criteria
- Clear communicator with excellent writing, report writing and presentation skills; capable of constructing and delivering clear ideas and concepts concisely and accurately for diverse audiences.
- Skills for communication on complex matters and difficult situations, requiring persuasion and influence.
- Skills for nurturing key relationships and maintaining networks.
