Job overview

Are you looking for an opportunity to work within a Primary Care Mental Health setting and develop your skills as a band 5 nurse, or are you soon to be qualified.

Our friendly and dynamic team is currently made up of both band 5 and 6 staff, some of whom are NMPs, working across the South locality and based in Park House Stroud.

We require forward thinking individuals who have an interest in working within a Primary Care context within local GP Surgeries and other GHC venues, offering assessment, consultation and support to Primary Care Practice staff. 

Main duties of the job

Our service provides short-term interventions and signposting to other community services which are based on health promotion, recovery and socially inclusive principles. The successful candidate will be expected to meet the mental health needs of the service users in Primary Care with through joint working, education and training.

The service currently has access to a range of Psychological interventions that form part of the Improving Access to Psychological via our Let’s Talk branch of the MHICT service and we work closely with our fellow colleagues in the secondary services.

You will be supported by our band 6 nurses whilst being able to access Supervision and workload management as per our Trust policy.

Detailed job description and main responsibilities

  • To participate in the triage and assessment of people with mental health problems on the telephone and face to face in surgery settings.
  • To use an assessment script to manage these appointments. Also using the clinic diary and recording on electronic patient record systems including IAPTus .
  • To provide web based and postal information and options regarding short term interventions for those people with common mental health problems, within clusters 1, 2, 3 and some cluster 11 clients.
  • To assist with the identification of those people who need specialist mental health services and signpost to other community based third sector services when appropriate.
  • To contribute to the smooth handover of mental health care between primary and secondary services and vice versa.
  • To ensure recovery skills based approach and social inclusion in line with the 10 essential capabilities.
