An exciting new opportunity has been created for 2 enthusiastic registered nurses to provide health advice and information for children and young people aged 0-25 with Special Educational Needs and / or Disability (SEND) as part of their Education, Health and Care (EHC) needs assessment, or as contribution towards their EHC Plan Annual Review.   

This post has responsibilities across Gloucestershire and will facilitate a predominantly virtual / remote service, but will also in-reach into educational settings across the county.  

The post holder will be required to provide an expert clinical opinion, signposting support and guidance to the child/ young person/ their family, whilst acting as a conduit for information exchange between the child/ young person/ their family and the local authority.

A key feature of the role will be to work alongside education settings and education colleagues (such as SENCo’s), to ensure that Gloucestershire’s Graduated Pathway is accessible and being utilised to provide early help to all children, young people and their families with additional needs.

This is a new post, which will require the post holder to be visionary and creative when developing and implementing new ways of working. 

Main duties of the job

• To provide clinical assessment and interpretation of health information from a number of sources for children and young people who are not receiving an active episode of care from community health services. This will also involve liaison with other health and education professionals outside of the Children’s Services, for example the school SENCO, GP or acute hospital specialist team.
• To provide written and/or verbal reports that present clinical information clearly and accurately that is accessible for both child and young person, their family / carers and other professional colleagues.
• To provide multiagency training for agencies working with children and young people with SEND about the Gloucestershire graduated pathway with a focus on health.
• To provide specialist advice and support to health professionals
• To be innovative and creative in managing demand-capacity issues.
