Job overview

An exciting new opportunity has been created for 2 enthusiastic registered nurses to provide health advice and information for children and young people aged 0-25 with Special Educational Needs and / or Disability (SEND) as part of their Education, Health and Care (EHC) needs assessment, or as contribution towards their EHC Plan Annual Review.   

This post has responsibilities across Gloucestershire and will facilitate a predominantly virtual / remote service, but will also in-reach into educational settings across the county.  

The post holder will be required to provide an expert clinical opinion, signposting support and guidance to the child/ young person/ their family, whilst acting as a conduit for information exchange between the child/ young person/ their family and the local authority.

A key feature of the role will be to work alongside education settings and education colleagues (such as SENCo’s), to ensure that Gloucestershire’s Graduated Pathway is accessible and being utilised to provide early help to all children, young people and their families with additional needs.

This is a new post, which will require the post holder to be visionary and creative when developing and implementing new ways of working. 

Main duties of the job

  • To provide clinical assessment and interpretation of health information from a number of sources for children and young people who are not receiving an active episode of care from community health services. This will also involve liaison with other health and education professionals outside of the Children’s Services, for example the school SENCO, GP or acute hospital specialist team.
  • To provide written and/or verbal reports that present clinical information clearly and accurately that is accessible for both child and young person, their family / carers and other professional colleagues.
  • To provide multiagency training for agencies working with children and young people with SEND about the Gloucestershire graduated pathway with a focus on health.
  • To provide specialist advice and support to health professionals
  • To be innovative and creative in managing demand-capacity issues.

Detailed job description and main responsibilities

  • To participate in local clinical governance/audit/research projects by providing data as required, and may, with support, contribute to departmental audit/research and design.
  • To actively participate in co-production and information sharing with all of the Gloucestershire local area parent /carer forums and young people’s forums.
  • To use the electronic patient management systems to access and record information and generate reports adhering to GDPR guidelines.
  • To act in a transparent manner, reflecting and challenging own practice with peers.
  • Able to adapt practice to meet individual circumstances, and to meet the demand-capacity requirements of the service.
  • To manage difficult conversations and complex situations as they arise and seek organisational support as required.
  • To maintain a clinical professional registration with an approved Body.
  • Contribute to performance improvement, taking a lead for identified areas where agreed.

 

Person specification

Qualifications

Essential criteria
  • Registered Nurse or Midwife
  • Knowledge of national guidance and policy related to SEND, including the Children and Families Act
  • Evidence of teaching or training professional groups

Experience

Essential criteria
  • Ability to work to tight and often changing timescales / deadlines
  • The ability to identify and act upon adverse clinical signs

Skills and Attributes

Essential criteria
  • Ability to act upon incomplete information, using experience to make inferences and decision making
  • Ability to analyse complex issues where information is drawn from multiple sources.

Other

  • The ability to move between sites within the Trust or county (including schools), as demanded by the job role
