·         Work as a member of the multi professional team providing clinical care to patients with a diagnosis of learning disability from the age of 18years and above as delegated by the Registered Nurse.

·         Ensure a high standard of patient centred care.

·         Promote self- management of health needs and adopt a positive risk based care approach.

·         Complete relevant forms of care, under the direction of a Registered Nurse.

·         Nursing Associates can assist in the delivery of care to patients, with a multiple combination of multiple chronic conditions, mental health issues, medication related problems and social vulnerability (defined as complex care).

Main duties of the job

•  To act as lead clinician those service users identified and directed by senior practitioner assessing service user’s needs, developing and implementing care plans, including risk assessments and carers assesments.to participate in induction and support of new staff and students and to co-supervise junior staff.
• Support patients with all elements of the nursing process and be able to assist the team in the on-going assessment, planning, management and evaluation of care. 
• To plan and organise own activity, prioritising workload appropriately in order to make the best use of time and feeding back to the team as appropriate. 
• To input all activity with service users onto RIO or equivalent data in order to enable the collection of accurate statistics, as required by the Trust.
• Recognise issues relating to safeguarding vulnerable children and adults to report any problems or raise concerns to appropriate registered care professionals. 
• Demonstrate understanding of caring for patients with particular conditions for example mental illness, learning disabilities and autism. 
• Signposting patients to services which may be of benefit to their wellbeing, promoting independence and capability with their own network and community.
• Signposting patients to services which may benefit of their wellbeing, promoting community connections and make the best use of local asset. 
• To administer medications in accordance and with NMC code and Trust policies.
