We are seeking a dedicated nurse to be part of our team and to assist in delivering safe, compassionate care to all our patients. The role is a 12-month fixed term cover.

The SSN role is fast past and there are times within the working day that you will be lone working.  This requires a level of risk assessment, which can only be carried out effectively by solid triage skills and prioritising patient care.  The nurse will need to be calm, methodical and resilient as this role can often be challenging and time pressured. It is essential that the applicant is substantially qualified and experienced in acute medicine 

Using excellent nursing skills with outstanding interpersonal skills, endurance and empathy you will make both staff and patients feel cared for and supported. You will provide attention to detail, be a problem solver and care for our patients and families. You will be an integral part of the multi-disciplinary team and have great learning and development opportunities.

To assure high quality, patient-centred care by providing expert emergency clinical practice, advice, education, and effective nursing management of people who have experienced stroke.

To be a knowledgeable practitioner in the management of stroke patients and work as an autonomous practitioner demonstrating the skills of assessment, examination, diagnosis, and treatment.

Supporting compliance of local, national audit documentation and the delivery of the National Stroke Strategy.

 

Prif ddyletswyddau'r swydd

To provide a specialist stroke service to patients within GHNHSFT requiring practice that is led by higher levels of judgement, decision making and discretion by keeping informed of recent developments in stroke care and management.

To provide a specialist stroke service to patients within acute and rehabilitation phases of their condition.

To assist in the preliminary assessment of patients for potential thrombolysis and thrombectomy.

To communicate effectively and actively participate in Multi-Disciplinary Team (MDT) meetings to aid patient care, joint treatment sessions, case conferences and specialist interest group meetings.

To communicate effectively with patients and carers, potentially complex, sensitive, or distressing information to provide patient centred care. Patients may have communication, perceptual or cognitive/understanding difficulties/disability.

To maintain clear, accurate, concise, and contemporaneous patient records in line with departmental and professional standards and include relevant outcome measures.

Provide health promotion for patients in line with their clinical requirements.

Job responsibilities

To facilitate the rapid transfer of all stroke and TIA patients requiring admission to the hyper-acute stroke unit (HASU) and foster supportive and positive working relationships with other departments.

Use judgement skills and knowledge and nursing, medical and therapeutic interventions to devise a specialist, individualised treatment and management plan encompassing secondary prevention and reflecting evidence-based practice.

To support the rapid assessment, scanning and delivery of thrombolysis assessment to appropriate patients according to agreed protocols and national guidance within a 24/7 service.

Provide dysphagia screening within agreed protocols.

Ensure the safe delivery of thrombolysis and post thrombolysis care, including prompt response to complications, including protocols for referral to tertiary centre as necessary.

To support and educate HASU staff to care for the thrombolysed patient and manage complications 24-48 hours post thrombolysis.

To expedite and provide advice regarding the deteriorating stroke patient and onward referral to tertiary centre and treatment options, as appropriate.

Actively participate in the Consultants ward rounds.

Provide further review of patients as required on HASU or outlying wards. Co-ordinate further investigations and assessment as required.

To co-ordinate stroke review and liaison for patients on stroke or other wards to optimise early stroke care.

  • Recognised Teaching and Assessing qualification.

Experience

  • Enhanced knowledge and experience in the assessment of the acutely ill patient / stroke patient

  • Evidence of involvement in audits / quality improvement projects and working collaboratively with team colleagues

Knowledge

  • Awareness of the Sentinel Stroke National Audit Programme (SSNAP) and understanding of measuring outcome related to stroke care
  • Competency in extended clinical roles such as cannulation and venepuncture
  • Ability to switch between different activities and unpredictable demands to prevent hospital admissions and accelerate hospital discharges.
  • To comply with professional codes of practice including revalidation, professional guidelines, departmental policies, and own performance appraisals
  • Evidence of leadership experience in an acute environment and Enhanced IT skills to support electronic caseload management
  • Excellent communication and interpersonal skills.
