Are you a Registered Adult Nurse with a passion for providing specialist continence and urology care?

An opportunity has arisen to join our dynamic Community Continence and Urology Team. We are a multidisciplinary team dedicated to delivering high-quality services across North East Essex.

Our service operates between 0800 – 1800, 7 days a week, providing essential care to patients in their homes and in nurse-led clinics, aiming to restore maximum independence and promote self-management in daily living.

Main duties of the job

As a vital member of our team, you will:

Case Management: Manage a caseload of patients with continence and urology needs.

Specialist Care: Assess, treat, and review adults with continence needs and/or bladder and bowel dysfunction, ensuring individualized care plans are agreed upon with patients and/or their carers and family members.

Nurse-Led Clinics: Work within nurse-led clinics and patients' homes, accepting referrals from healthcare professionals and associated agencies.

Care Planning: Plan and implement care programs for the specialist management of continence problems. 

Support and Counselling: Provide psychological support and counselling to adults and their carers/family members, addressing their identified continence needs.

Catheter Management: Assess and implement care plans for complex catheters and manage nurse-led community catheter clinics.

Specialist Advice: Act as a point of contact for specialist advice, information, and support to healthcare professionals.

Training and Education: Provide continence, urology, and catheter training to residential homes, MDT, and the wider healthcare service as required.

Clinical Practice Improvement: Implement evidence-based changes in clinical practice to enhance the standard of care and health outcomes for adults with continence problems.

Collaboration: Establish positive working relationships with other healthcare professionals to promote a seamless service.
