Job overview

We have exciting openings in the Diabetes Team at Colchester Hospital. The Diabetes team is dynamic and forward thinking & have expanded to provide an integrated diabetes service.

You will be  working as part of this multidisciplinary team, participating in the delivery of integrated diabetes care for people with diabetes across ESNEFT.

Main duties of the job

You will act as a recognised clinical expert in all areas of diabetes care and management.

You will provide expert education and support for adult patients and their carers with regard to all areas of diabetes care and management Trust wide.

You will work as a practitioner within a multidisciplinary team, providing a continuous service for patients diagnosed with all forms of diabetes, providing holistic care, advice and support in both in-patient and out-patient settings.

You will act as mentor and provide expert knowledge and advice, clinical supervision and clinical governance to ensure patient safety.

Experience

Essential criteria
  • • Proven post registration experience
  • • Proven experience at Band 5
  • • Experience of developing, training and education in clinical practice

  • • Evidence of audit experience

Qualifications

  • Current NMC registration
  • Demonstration of personal and professional development
Desirable criteria
  • A recognised Diabetes Course
  • Preparing for mentorship course

Knowledge

  • • Able to demonstrate a wide knowledge of current trends in nursing and application to practice

  • Experience in working with patients with diabetes and being involved in diabetes related treatment decisions
