At Rye, Winchelsea and District Memorial Hospital we offer outpatients, rehabilitation and intermediate services. At Uckfield Community Hospital we provide day surgery and outpatient care.
In the community, we deliver services that focus on people with long term conditions living well outside hospital, through our integrated locality teams working with district and community nursing teams. Community members of staff also provide care to patients in their homes and from a number of clinics, health centres and GP surgeries.
Job overview
This is an exciting and challenging post in which you will work as part of a team caring for patients with Heart Failure in the community. This work will take place in an outpatient clinic/domiciliary setting and you must have a full UK driving licence.
You will support patients who have a diagnosis of Heart Failure; educating them and enabling them to monitor their symptoms and ensuring they are stable whilst helping to optimise their medical management.
You will work closely with East Sussex Community Health Services Staff and East Sussex Healthcare NHS Trust and in collaboration with Consultants, Physicians and Cardiologists. You will participate in regular multi-disciplinary meetings.
You will manage your own caseload supported by your band 7 line manager. You will be involved in developing an effective, streamlined and efficient service for the care and management of patients.
Main duties of the job
The post holder will have a clinical role with responsibility for the assessment, planning and co-ordination of care to people with diagnosed heart failure. They will manage their own caseload with support from their band 7 line manager. They will perform clinical assessments, review and optimise medication and support positive lifestyle changes through education. The aim will be to prevent acute exacerbations and unnecessary hospital admissions and support the Virtual Ward initiative.
S/he will continuously develop the Heart Failure Service in collaboration with the Specialist Nurses, Heart Failure Consultants, GPs, Community Matrons and other Community Healthcare staff to effectively manage heart failure patients at home. Support the delivery of education programmes, support groups and rehabilitation groups for patients, carers and healthcare professionals. Be a resource in an advisory role on all aspects of Heart Failure.

Qualifications
Essential criteria
- Registered General Nurse
- Relevant first level degree or working towards qualification
- ENB998 or other training / mentorship qualification / equivalent skills
- Evidence of post-qualification training and development in cardiology nursing
- Knowledge of clinical/strategic issues relating to the management of heart failure ( NICE guidelines for heart failure, NSF for CHD
Desirable criteria
- Specialist cardiology nursing qualification or other relevant post graduate qualification
Experience
Essential criteria
- Significant recent experience in managing patients with coronary heart disease at a band 6
- Experience of assessment and care planning for specialist patient group
- Experience of ECG recording
Skills
Essential criteria
- Enhanced clinical skills in specialist area.
- Proven patient assessment skills
- Demonstrates knowledge of medications used in the management of the heart failure patient
- Knowledge of clinical/strategic issues relating to the management of heart failure such as NICE guidelines for heart failure and the NSF for CHD
